Terros Health is hiring a Patient Navigator at our 27th Avenue Health Center in Phoenix, AZ.
Full-Time, Employed
Location: 27th Ave/ Indian School Rd
Monday-Friday, 8am-5pm
No Weekends
Education/ Experience Requirements:
A High School Diploma/GED, Associate’s Degree, or Bachelor’s Degree (Not in the Field of Behavioral Health)
Plus a Minimum of 1.5 years Experience Working in the Behavioral Health Field, OR
Bachelor’s Degree in the Field of Behavioral Health
1-3 Years of Experience in Health Care Services and/or Customer Service
Lived Experience in Mental health or Substance Use Recovery and Peer Employment Certification Required;
or Ability to Obtain Peer Employment Certification Within 90 days of Assuming Role

Position Summary:
The Patient Navigator is part of the frontline team interfacing with patients in Terros Health Centers. This position helps create energy and excitement around Terros Health and guides patients to the right solutions and services. The position performs a variety of duties which include, but are not limited to, greeting individuals, appointment scheduling, intake interviews, gathering information and paperwork. The Patient Navigator is dedicated to delivering an excellent patient experience, discovering patients’ needs and turning every visitor into a satisfied and loyal Terros Health patient. This position reports to the Practice Manager/ Practice Supervisor.
