ChristianaCare's Outpatient Neurology department is looking to hire a Full Time Senior Social Worker I to support the practice at the Health Care Center in Newark!

The Senior Social Worker will be responsible for enhance the quality of patient care within the Neurosciences Service Line by providing assessment, treatment, referral, community connections, and educational services for the Multiple Sclerosis, Movement and Epilepsy Populations.

Work Schedule:

Monday- Friday: day shift

Key Responsibilities:

Performs comprehensive assessments detailing psychosocial and environmental factors, and their effects upon the client's health and well-being.

Collaborates effectively with multi-disciplinary team in developing care plans consistent with evidence-based care guideline that sustain longitudinal social, emotional and physical health improvement and stability.

Partners with individuals and families to create a plan to address the behavioral health issues that serve as barriers to health.

Makes appropriate referrals for clinical intervention as warranted.

Collaborates with providers and colleagues throughout the continuum to ensure smooth transitions of care.

Facilitates psycho educational and support groups as warranted by population needs.

Provides education and resources on Macro, Mezzo and Micro-level of care across the spectrum.

Advocates for clients when needed, empowering them to achieve their goals, including individual and or class inequities, inadequate or non-existent hospital or community resources (i.e.: insurance benefits, housing, benefit applications, verification documents, etc.).

Facilitates coordination of care and focuses on access to care improvements.

Qualifications:

Master's Degree in Social Work (MSW) from accredited graduate school of Social Work required.

1 - 3 years of recent experience within a health care setting; community work preferred.

LMSW or LCSW as required by the state of Delaware.

Direct experience in transition planning, insurance authorization and arranging community services for complex populations.
