Case Manager – Minneapolis, MN Hekima Cultural and Consulting Services is a social services agency that provides person-centered services to individuals with disabilities and organizations. It was established in 2018 to address the need for holistic, strength-based, culturally centered services for all Minnesota communities. The Company develops connections with the people they serve and assists them in acquiring the res ources they need to achieve their goals. This full-time hourly positions is located at 1200 Mendelssohn Ave , Golden Valley MN 55427 and 1549 Livingston Ave E Suite 106, West St. Paul MN 55118 Position Overview:
As a Case Manager, you will demonstrate your commitment to your clients and Hekima Cultural and Consulting Services by performing various job duties. The Case Manager will provide person-centered assessments, planning, facilitation, and advocacy for options and services to meet the needs of children, the elderly, or adults with disabilities who receive Waiver services. Also, monitor individuals' health and safety needs and intervene, when necessary, to ensure the effectiveness of services provided and collaborate with other professionals. In this role, you will be responsible for:
-           Providing person-centered case management services that assist individuals who receive Home and Community Waiver Services in locating and gaining access to needed waivers and other state plan services while meeting the person’s medical, social, housing, employment, and other goal areas.
-           Developing and implementing, with the individual’s involvement, a person-centered Coordinated Services and Support Plan (CSSP) that is based on the MnCHOICES assessment results and within the county-approved allocation for the person.
-           Evaluating, facilitating, referring, authorizing, and monitoring services and individual progress related to established goals and objectives and working with the person to adjust service plans as necessary.
-           Participating in team meetings, agency staff meetings, county meetings, and training.
-           Actively participate in ongoing professional growth and development; maintain appropriate professional behavior and participate in appropriate supervision.
-           Obtaining the necessary CEUs to maintain professional licensure status.
-           Other tasks and responsibilities as assigned.

-           You know how to prioritize. To qualify, you will bring:
-           Bachelor’s degree or higher in Social Work, a Human Service field, Psychology, or Sociology. Minnesota Social Work Licensure required.
-           Must have prior experience working with children, seniors, or disabled adults in a community-based setting, including experience with assessing strengths and needs, providing information, referrals, and follow-up to other community resources.
-           Must have access to reliable transportation, a valid Minnesota driver’s license and current insurance as travel within the County and surrounding areas for home visits is required.
-           Two years of experience working with Minnesota Medicaid Waiver programs (Alternative Care, Elderly Waiver, CADI, DD, and BI Waivers)is preferred. About Us
