Staten Island CMCT will respond to a child/young adult in crisis at home, school, or elsewhere in the Staten Island community. Moreover, Staten Island CMCT staff can provide mental health engagement, intervention and follow-up support to help overcome resistance to treatment. Position Overview: The Social Worker is responsible for supplying necessary clinical interventions within the realm of the Rapid Response Mobile Crisis program. Provide culturally competent and sensitive services. As part of a therapeutic team within the Agency, work closely with the divisions‘ medical, psychiatric and psychology staff, to ensure the appropriate treatment interventions for the client. RESPONSIBILITIES include but not limited to: Outreach to families in crisis in the community within 2 hours of receiving the referral. With the help of the Crisis Family Advocate Social Workers help to de-escalate the child in crisis as well as helping families manage emotional reactions to child’s crisis. Coordinate all services for consumers, including day programs, behavioral health, medical care, transportation, summer programs (when applicable), recreational activities, and community inclusion (Person Centered Planning) Liaise between Department and consumer‘s family members, and relevant community members. Maintain records for all contacts with clients and their families. Link clients to community resources that support their behavioral health goals. Conduct meetings to ensure proper treatment, progress in program and coordination of goals. Participate in audit preparation. Work with program supervisor on intake process for new consumers. Work with Family Advocates to enhance communication and cooperation between them and consumer families. Complete paperwork in a timely manner. ·Other duties as assigned. Core Competencies : ·Knowledge of mental illness, serious emotional disturbance and substance use disorders. ·Ability to document assessments, plans and interventions Educational / Training Required : ·Licensure (LMSW, LCSW) in Social Work ·Valid NYS Driver’s License Experience Required / Language Preference: Minimum of one year of relevant experience with similar population. Computer Skills Required : Familiarity with Outlook and Excel Work Environment / Physical Effort: ·To perform the essential functions of this job the candidate must be able to travel within New York City carrying equipment such as a notebook, forms, laptop, mobile hotspot and cell phone weighing up to approximately 10 pounds. · Risks/hazards associated with the position are those which may be encountered travelling around New York City.
