JOB SUMMARY

The VASH Case Manager is responsible for the case management of individuals and families that utilize the VASH Voucher through the VOACC contract with Veteran Affairs. This position shall be supervised by the Clinical Case Manager to ensure best practices for assisting individuals/family in their journey through homelessness to housing. The Case Manager will develop a strength-based relationship that promotes the residents ability to identify barriers and solutions to stabilized housing. The Case Manager will ensure that each resident is provided referrals and supports to employment, housing resources, and various other Individual Program Plan goals to assist in the goal of establishing stabilized housing. The Case Manager promotes the mission, vision and core values of the organization and is responsible for providing leadership that supports and promotes the strategic direction of VOACC in the area of Case Management support services.

PRINCIPLE ACTIVITIES :

  • To accurately complete required data entry and program reports in a timely manner, under the direction of the Clinical Case Manager.
  • To coordinate all incoming and outgoing correspondence; dating and stamping all documents as appropriate; maintain comprehensive written records of events during shift; ensure residents sign in/out log, as well as all other log books and resident forms, are being used properly.
  • To maintain an adequate supply of forms; modifying forms as needed or directed.
  • To monitor all office equipment, reporting malfunctions, and coordinating the repairs as directed.
  • To order and disburse office supplies as needed, with approval of Clinical Case Manager.
  • To transcribe, type, and distribute the minutes of meetings as directed.
  • To participate in meetings and staff training as directed.
  • Participate in the application review and comprehensive intake process of assigned potential residents.
  • Complete residents Individualized Program Plan from intake through discharge.
  • Establish and maintain effective, positive working relationships with Veteran Affairs staff, as well as local and professional community agencies and resources.
  • Assist in ensuring complete program records are generated and maintained in accordance with policies and procedures and contractual and regulatory requirements.
  • Act as liaison between the programs and funding/regulatory agencies.
  • Provides strength based, solution-oriented case management services to residents
  • Provide on-going support/oversight to ensure service delivery is in accordance with the HEARTH act and client centered and focused.
  • Attend and participate in all work-related meetings and training as required or needed.
  • Complete screening reports and follow program guidelines for informing callers of space availability and available resources.
  • Complete initial intake packets and orient new residents.
  • Be familiar with the Client Handbook, Operations Manual, Policies and Procedures, and Program-specific documents.

Duties may also include:

  • Ensuring the appropriate application of HIPAA, PHI and all program compliance; policies and procedures.
  • These skills will be applied to maintaining client files, submitting all program all client and program level reports to both VOACC Office Excellence (OE) and VA.
  • Additional duties may include supporting the Clients and staff, with identifying resources available to support programs through community soliciting, collecting, organizing, and distributing donations; and maintaining appropriate records concerning volunteers and donations.

  • To maintain accurate and complete records and reports.
  • To maintain efficient support for residents.
  • To provide an effective link between Program management staff.
  • To present a good public image of the VOACC and its program by providing accurate , useful information in a positive and timely manner .
  • The successful candidate must possess a Licensed Social Work Degree or related field with one to three years of case management or client record keeping experience.

  • Knowledge/Experience with the Housing First Model is preferred.

  • Use of computers include knowledge and application of software to electronically create and submit correspondence and program reports; electronic media and meeting platforms, (i.e. Word, Excel, Microsoft Teams, Zoom Skype etc.).

  • Candidates must also maintain knowledge and proficiency in grammatical syntax and general accounting.

    • The physical ability to travel to assigned locations, stand, stoop, bend, reach, pull, push, lift, grasp, climb, talk, see, hear and perform basic and light home maintenance activities, and operate office equipment. ­

    • Move and lift light objects up to 30 pounds such as mail, supplies, files, and equipment.

    • Operating office equipment requiring continuous or repetitive hand/arm movements.

    • The ability to remain in a sitting position for extended periods of time.
