As a Clinical Coordinator for our CSIDD/ Start program you will provide groundbreaking crisis intervention and prevention services for individuals with intellectual and developmental disabilities and mental health needs in Brooklyn.

We are willing to supervise towards clinical hours required for licensure New Graduates are encourged to apply.

This is an exciting opportunity for new and seasoned clinicians to learn and practice the START model - a comprehensive, collaborative bio-psychosocial approach to intervention aimed at mitigating crisis occurrences.

CSIDD/Start offers crisis prevention and response services to people who have both developmental disabilities and complex behavioral needs, as well as to their families and those who provide supports. The CSIDD/Start multidisciplinary treatment team is a dynamic, inspiring and supportive group of professionals who are dedicated to helping families support the people they love and care for by using a systematic and effective approach. The team prides themselves on building relationships and supports across service systems to help people remain in their homes and communities, and enhance the ability of the community to support them.

ESSENTIAL DUTIES & RESPONSIBILITIES:

Outreach

  • Provide outreach visits to the individuals’ home, day and vocational settings regularly.

  • Conduct consultative sessions to Include the individual, their family/caregiver, and all active members of their system of support.

  • Maintain an active caseload of individuals referred for CSIDD/Start Services (variability occurs depending on activity level of cases).

Assessment

  • Complete individual assessments under the supervision of a Medical Director.

  • Develop Comprehensive Service Evaluations, Cross-Systems Crisis Prevention and Intervention Plans.

  • Generate intake/assessments, admission/discharge summaries for resource center admissions or inpatient hospital admissions and any other applicable documentation of services provided.

  • Document all services contemporaneously with the date of service, maintaining up to date records.

Planning

  • Develop individualized clinical crisis plans and treatment plans based on the results of clinical assessments.

  • Consult with the team to identify which CSIDD/Start services are appropriate for the individual supported.

  • Ensure the coordination of support meetings and crisis plan development for individuals enrolled in CSIDD/ Start services.

  • Provide on-call support on a rotational basis as part of a CSIDD/Start team.

Training & Networking

  • Develop materials and present cases at Clinical Education Team Meetings as assigned by the Director or designee.

  • Participate in the Center for START Services National Network, which brings together projects from across North America through an online resource center, conference calls, select study groups and a meeting at an annual conference as recommended

  • Perform other duties as required.

REQUIRED EDUCATION AND EXPERIENCE

  • A Master’s Degree in social work, psychology, counseling or other human service field

  • Preferred but necessary 2 years' experience working with people with IDD and behavioral health needs

  • Independent and organized work skills with strong communication and writing skills necessary

  • Willing to work to complete START Coordinator certification within 12-18 months following employment.

  • Willing to travel throughout Brooklyn.
