Starting Rate: $23.50/hour
High School Diploma/GED Mon-Fri 9am-5pm
Provide intensive case management support to families across the Central Massachusetts region
Provide case management support to families across the Metro Boston region and supervision and oversight of the IFFS program.
- Individual will be responsible for working with and providing intensive case management to families of children and young adults (3-22 yrs)
- Responsibilities will include home visits to assist families in identifying challenges, finding and presenting resources, gathering information, securing referrals and supporting the family in navigating multiple state systems and community resources to access services
- Additional languages a plus.
- Have a bachelor’s degree and experience in the Human Services field, or, in the alternative, significant personal experience with the ID/DD and medically fragile population
- Two years’ experience working with people with developmental, behavioral and mental health challenges (may be substituted with personal experience navigating supports for relatives)
- Proficient computer skills including Microsoft Office Suite as well as the ability to quickly learn HMEA’s software systems
- Valid Driver’s License and ability to travel throughout the Central Massachusetts region for home visits
- Ability to work independently and accountability are required
- Additional languages a plus
