Job Description - Physician (PACT)

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application. To qualify for this position, you must meet the basic requirements as well as any additional requirements (if applicable) listed in the job announcement. Applicants pending the completion of training or license requirements may be referred and tentatively selected but may not be hired until all requirements are met. Currently employed physician(s) in VA who met the requirements for appointment under the previous qualification standard at the time of their initial appointment are deemed to have met the basic requirements of the occupation. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Degree of doctor of medicine or an equivalent degree resulting from a course of education in medicine or osteopathic medicine. The degree must have been obtained from one of the schools approved by the Department of Veterans Affairs for the year in which the course of study was completed. Current, full and unrestricted license to practice medicine or surgery in a State, Territory, or Commonwealth of the United States, or in the District of Columbia. Residency Training: Physicians must have completed residency training, approved by the Secretary of Veterans Affairs in an accredited core specialty training program leading to eligibility for board certification. (NOTE: VA physicians involved in academic training programs may be required to be board certified for faculty status.) Approved residencies are: (1) Those approved by the Accreditation Council for Graduate Medical Education (ACGME), b) OR (2) Those approved by the American Osteopathic Association (AOA),OR (3) Other residencies (non-US residency training programs followed by a minimum of five years of verified practice in the United States), which the local Medical Staff Executive Committee deems to have provided the applicant with appropriate professional training and believes has exposed the physician to an appropriate range of patient care experiences. Residents currently enrolled in ACGME/AOA accredited residency training programs and who would otherwise meet the basic requirements for appointment are eligible to be appointed as "Physician Resident Providers" (PRPs). PRPs must be fully licensed physicians (i.e., not a training license) and may only be appointed on an intermittent or fee-basis. PRPs are not considered independent practitioners and will not be privileged; rather, they are to have a "scope of practice" that allows them to perform certain restricted duties under supervision. Additionally, surgery residents in gap years may also be appointed as PRPs. Proficiency in spoken and written English. Reference: VA Regulations, specifically VA Handbook 5005, Part II, Appendix G-2 Physician Qualification Standard. This can be found in the local Human Resources Office. Physical Requirements: Moderate lifting (15-44 lbs.); Light carrying (under 15 lbs.); Straight Pulling (up to 2 hours); Pushing (up to 2 hours); Reaching above shoulder; Use of fingers; Both hands required; Walking (up to 3 hours); Standing (up to 3 hours); Kneeling (up to 1.5 hours); Repeated bending (up to 2 hours); Near vision correctable at 13" to 16"; Far vision correctable in one eye to 20/20 and to 20/40 in the other; Hearing (aid permitted); Clear speech. Contract Buy Out Program: This position may be eligible for the Physician / Provider Contract Buy Out Program (CBOP), which authorizes VHA to buy out service contracts (Locum agreements, loan repayments, sign-on bonus contracts, Physician-owned practices) in exchange for employment at certain rural or highly rural facilities for a period of obligated service (minimum 4 years). For additional information and eligibility requirements, contact the VHA Contract Buy Out Program Manager at Duties of the physician: A primary care physician will be responsible for delivery of primary care to patients and will manage scheduled and unscheduled patients in the outpatient clinic setting. The primary care provider is an integral member of the PACT teamlet and is expected to work closely and coordinate patient care with other PACT teamlet members, including nursing and clerical staff. Primary care physicians will participate in daily huddles with other teamlet staff and participate in primary care systems redesign projects. A primary care provider in the outpatient setting is assigned a panel of patients to manage under PCMM guidelines; recommended panel size at the division level is calculated based on support staff and room availability. Each provider's panel size is specific to their practice and setting. Alternative or collateral duties may include management of a subsection of primary care patients, the subsection may be defined either by a disease status (e.g. spinal cord injury patients), or demographic status (women veterans, OIF/OEF veterans, employees etc). Typically these duties are designated at the time of joining but may be assigned in the course of employment in response to new initiatives. Primary care physicians may have responsibility for supervision of or collaboration with primary care mid-level providers. Physicians are expected to provide clinical oversight and to participate in peer review of other primary care providers through periodic chart reviews. Primary care physicians will be responsible for supervision, education and evaluation of internal and/or family medicine residents and medical students in the outpatient primary care setting. Primary care physicians will act as surrogates for absent primary care providers when needed, to include review of CPRS view alerts. Primary care physicians will regularly attend primary care staff meetings, departmental meetings and medical staff meetings. Typical work hours are Monday to Friday 8-4.30 PM, and a full time provider is required to work a minimum of 80 hours per pay period. All physicians need to follow departmental and VHA policies for leave and attendance and will work within Title 38 appointment guidelines. VHA guidelines do not permit overtime pay for physicians, however comp time may be granted for physicians who are asked to work outside their normal assigned tour of duty. Leave should be requested as far in advance as possible and with at least sixty days notice in order to minimize patient cancellations. All unscheduled absences must be properly communicated, and patients should be rescheduled within time frame as specified in department's policies. Timely documentation (notes and encounter forms) is necessary in all areas in accordance with facility policy. All administrative work should be completed in a timely fashion.
