Brunswick Hospital Center (BHC), is a 146-bed private, acute-care psychiatric hospital that is licensed by The New York State Office of Mental Health and accredited by The Joint Commission and Centers for Medicare & Medicaid Services. Brunswick Hospital Center caters to a diverse population throughout the NY Metropolitan region and is located 45 minutes from Manhattan and in close proximity to all major expressways and the LIRR/local bus service.
We’re currently seeking full time, part time & per diem Pharmacists to monitor and evaluate all aspects of pharmaceutical care, clinical services to patients and to other healthcare providers. Recommendations are made to ensure safe and proper dosing, administration and monitoring of drug therapy to maximize the quality of care given while minimizing cost.
Primarily, you will:
- Review individual patient orders to optimize therapeutics to ensure safety and effectiveness.
- Using objective parameters and professional judgement, monitors and evaluates a patient’s medication regimen for efficacy, toxicity, side effects and outcome.
- Collaboratively manage medication therapy for individual patients by conveying relevant findings and making recommendations to other clinicians regarding a patient’s medication regimen
- Counsel and educate patients concerning their medication, address potential drug-drug and drug-food interactions, side effects and other information aimed at improving adherence and document the counseling in the medical record.
- Participate in the reconciliation of patients’ medications to ensure that the accurate and complete medication list is communicated throughout the continuum of care.
- Contact/confer with physicians if there are any questions or irregularities regarding patient orders.
- Report/document all known medication discrepancies or adverse drug reactions.
- Maintain all required records, both legal and hospital, including controlled substances, manufacturing, and packaging records.
- Maintain established departmental policies and procedures, quality assurance programs, safety and infection control standards and work toward achieving departmental key result areas.
- Weekly Antibiotic stewardship
- Clozapine REMS maintenance, reporting and follow-up of labs.
- Maintenance of Pyxis MedStations and daily filling/loading of medication into Pyxis.
- Participate in the development and implementation of QAPI programs with assistance and under the direction of the Director of Pharmacy.
- Assist with the implementation of the decisions of the Pharmacy & Therapeutics Committee.
- Assists in the training of pharmacists, pharmacy residents, pharmacy interns, pharmacy students, and pharmacy technicians. Attends and participates in departmental meetings, in-services, huddles, and departmental trainings as applicable.
- Understand patient safety goals and other quality metrics that affect area of practice. Report and follows up on medication or patient safety issues as needed; works to resolve problems in a manner that will improve patient safety and team efficiency.
- Provide formal in-service programs for the Nursing Department as requested by the Director of Pharmacy.
- The pharmacist will exhibit at all times while on duty an attitude of cooperation, teamwork, and courtesy in all interactions with other staff as well as patients and family of patients.
- The pharmacist will employ politeness and telephone etiquette with all staff, patients, pharmacies, outside vendors, and families who call the pharmacy seeking information and/or assistance.
We require:
· Graduation from an accredited college of pharmacy.
· Current NYS Registration as a Licensed Pharmacist.
· Hospital pharmacy experience preferred but not required.
· Basic data entry skills.
· Excellent attention to detail.
· Excellent interpersonal, and English verbal/written communication skills.
· Must be available to work: days, evenings, nights, weekends and/or holidays as required.
Salary $72.14-$79.35
