Fraser is seeking a Group Home Coordinator / Designated House Coordinator (DHC) to join our Community Living team. In this role, you will oversee the day-to-day operations of a group home while providing leadership, coaching, and training to staff. You will ensure compliance with Fraser policies and 245D regulations, support continuous quality improvement efforts, and help maintain a safe, supportive environment for both staff and the individuals we serve.
This hands-on leadership position includes working directly with individuals living in our group homes to support their personal goals, promote independence, and assist with daily living activities and personal care needs as required. A portion of your weekly schedule will be dedicated to providing direct support services.
Responsibilities:
Provide on-site supervision and leadership to staff at the group home or residential location.
Ensure compliance with 245D regulations and Fraser policies.
Support active treatment service development and implementation.
Lead continuous quality improvement initiatives at the location.
Serve as a primary point of contact for clients, families, and community partners.
Monitor and maintain accurate client and staff records, including documentation required by regulations.
Participate in emergency response and preparedness activities as needed.

Candidates must qualify based in one of the following three ways, as assigned by 245D, which includes a combination of work experience and education.
