Description

Supervisor - Day Program

Takes an active role in the development of the Day Habilitation Program and ensures compliance with OPWDD guidance and regulations. Supervises in the overall operation of the program and implementation of person-centered activities. Implements the day-to-day provision of active support and programming according to the individuals support plans. Promotes and protects the individuals' civil and personal rights as well as help to maintain the individuals' states of well-being, both physical and emotional. Promotes a positive and supportive work place for both staff and the individuals we serve. 

Essential Functions:

  • Develops and implements person centered activities which promote the program individual’s independence and interest 
  • Creates new groups and activities in an ever-evolving setting
  • Fosters active engagement
  • Models active engagement with the individuals for the staff, lead groups, etc
  • Supervises staff throughout the day, provides support and feedback as needed
  • Counsels staff around areas of concern and progressive discipline 
  • Completes and reviews documentation as needed related to incidents and injuries
  • Completes all HR documentation
  • Creates/supports the creation of the daily schedule of events and staff responsibilities
  • Manages staff call outs, vacation requests, issues that arise throughout the day, etc
  • Frequently writes concise in-services, including relevant supplementary materials, for the ever-changing diets, diagnoses, BSPs, plans of care, etc. of all of the individuals. Staff will read, discuss, and sign off, and supervisor will keep records of the in-services
  • Coordinate, schedule, and keep documentation on all staff trainings and ensures that everyone stays current
  • Conducts necessary trainings for all staff
  • Completes Core Competency Evaluations and observations
  • Attends Life Plan meetings and other team meetings as needed 
  • Coordinates volunteer sites and oversite 
  • Runs activities and groups when needed to support staff and individual ratio
  • Keeps track of petty cash and other program funds
  • Arranges trips and other activities i.e., movies, baseball games, aquarium, museums, shows, etc 
  • Corresponds with Care managers, residence managers, psychologists, families, clinical team, Day Program Director etc. regarding plans of action/care for all individuals
  • Reviews, completes and checks documentation to ensure services are being properly recorded. Assists with training and supporting staff in completing quality documentation. Including but not limited to precision care, life plans, CFA’s etc 
  • Maintains and orders program supplies
  • Maintains time in and time out logs for individuals’ attendance
  • Reviews and approves attendance
  • Implements progressive discipline 
  • Responsible for following through with any directives given by the Day Program Division Director in a professional and timely manner 

Knowledge Skills and Abilities

  • Knowledge of and ability to follow Agency and departmental policies and procedures
  • Ability to communicate effectively with all levels of staff    
  • Ability to maintain accurate records and attend to detail
  • Qualify as a QIDP 
  • Excellent communication, interpersonal and documentation skills and implementation of person centered activities
  • Ability to maintain confidentiality
  • Excellent interpersonal skills
  • Ability to work in a team environment
  • Excellent office, organizational and computer skills 
  • Ability to work independently and use appropriate judgment

Requirements

  • Bachelor Degree in the Human Services field required and 1 year of experience working with individuals with disabilities
