Position Summary:
The ACT Team Generalist Practitioner is a clinically trained and licensed professional
responsible for providing individual and group supportive therapy to consumers within the
Assertive Community Treatment (ACT) program. This role serves as a primary practitioner for a
caseload of consumers, delivering comprehensive, recovery-oriented services in collaboration
with multidisciplinary team members. The Generalist Practitioner conducts assessments,
develops treatment plans, and implements therapeutic interventions based on client needs and
goals. They contribute to the overall treatment team by participating in team meetings, case
conferences, and treatment planning sessions. The Generalist Practitioner maintains clinical
documentation and ensures compliance with regulatory requirements and quality standards.
ACT is an intensive, integrated, rehabilitative, crisis, treatment, and mental health community
support service provided by an interdisciplinary team to individuals eighteen (18) and over with
serious and persistent mental illness with dedicated staff time and specific staff-to-consumer
ratios.
Service coverage by the ACT team is required twenty-four (24) hours per day, seven (7) days per
week. Provides co-occurring disorders treatment using evidence-based and stage-appropriate
interventions.
Key Responsibilities:
Provide individual and group therapy to ACT team consumers, incorporating evidence-
based practices and recovery principles.
Conduct comprehensive assessments to determine client needs and develop personalized
treatment plans.
Collaborate with team members, including psychiatrists, nurses, case managers, and peer
support specialists, to coordinate care and ensure holistic support for consumers.
Monitor client progress, evaluate treatment outcomes, and adjust interventions as
necessary.
Participate in crisis intervention and management activities, including on-call
responsibilities as scheduled.
Maintain accurate and timely documentation of client interactions, treatment plans, and
progress notes.
Advocate for clients’ rights and collaborate with community resources to enhance service
delivery and support systems.
Qualifications:
Master’s degree in Social Work, Counseling, Psychology, or related field.
Current state licensure as a clinical practitioner (LCSW, LMFT, LPC, etc.).
Experience working in community mental health settings with individuals experiencing
severe and persistent mental illness preferred.
Knowledge of recovery-oriented practices, trauma-informed care, and evidenced-based
interventions.
Strong interpersonal skills and ability to work effectively within a multidisciplinary team.
