Description

Primary Role: Coordinate and implement activities, which provide opportunities for socialization, education, exercise, relaxation, and self-expression aimed at fulfilling basic psychological, social, and spiritual needs of participants. The Activities Coordinator will assist the physically-, emotionally-, and cognitively-impaired individual as well as caregivers to explore appropriate recreation and leisure pursuits in accordance with best practices and funding requirements. Provides case management services to physical and/or medically disabled adults and elderly participants enrolled in the ADHC or SC program. Implements and monitors services listed on the care plan.  

Duties and Responsibilities:

Individualized Support

  • Conduct needs assessments to ascertain physical, cognitive, and emotional abilities. 
  • Participants in Interdisciplinary Team Staffing and develops Individualized Support Plans 
  • Maximizes the social functioning of each assigned participant by providing individualized services that meet each participant’s needs. 
  • Regularly provides reality orientation to assigned participants or as needed  
  • Coordinates small groups for all participants 
  • Responsible for ensuring small group calendar is completed 
  • Coordinate outside the facility, ensuring the safety and well-being of each participant at all times. 
  • Ensures individualized support plans are being implemented and ensures progress 
  • Forge lasting relationships within community (clubs, groups, organizations, agencies, churches, individuals, etc.) 

Documentation

  • Documents delivery of services in form of progress notes at least once a month for the assigned participant in the data management system 
  • Obtains written consent from assigned participants as needed. 
  • Ensures Individualized Support Plans are documented daily.  
  • Maintain confidentiality of participant and facility records/information at all times 

Administrative

  • Responsible for accurate, complete and timely collection and submission of participant information for inclusion in CMS/AIS and/or other information systems as required 
  • Develop and maintain an active volunteer program through outreach, development, support, and organization in accordance with Clover policies. Encourage participation by family members, staff, visitors, and community groups and individuals.  
  • Secure all required volunteer documentation prior to placement. Assign and train volunteers and maintain documentation of volunteer hours. Acknowledge the contributions of volunteers for their service at special events. 
  • Maintain a pictorial scrapbook of activities and events for each year. 
  • Ensure engagement for participants and families  
  • Assist with calendar for residential engagement at Columbia Park Senior Living. 

Requirements

Essential Skills and Abilities:

  • Strong interpersonal skills, including ability to work well with others and communicate effectively with participants and staff.
  • Computer literate using MS Word, Excel, Power Point and Outlook and able to learn other software as needed.
  • Excellent organizational and time management skills, including ability to meet deadlines, and able to work independently.
  • Applies a thorough knowledge of grammar and spelling, and strong proofreading/editing skills.
  • Possesses efficient and accurate typing, data entry, filing, math, and office skills.
  • Able to obtain and maintain First Aid/Infant, Child, and Adult CPR certification.
  • Respects the integrity of each program participant, and maintains their right to privacy and confidentiality; maintains confidentiality with respect to personnel and proprietary agency information.
  • Able to meet deadlines, well organized, and able to work under pressure of multiple deadlines; demonstrates flexible and efficient time management and ability to prioritize workload.
  • Acts as a role model within and outside of the agency.
  • Adheres to agency policies and procedures.
  • Possesses a demonstrated understanding of the dynamics of poverty, oppression, sexism, racism and other forms of societal and individual prejudice that confront our staff, volunteers, program participants and community at-large. Possesses a proven ability to work effectively and respectfully with a broad range of people from diverse cultural, ethnic and socio-economic backgrounds.
  • Demonstrates a philosophy and value base consistent with the purpose and goals of Kingsley House, advocating a strengths and empowerment perspective and working towards increasing the capacities of program participants and their families.
  • Possesses the ability to work holistically, as part of a team, with staff, program participants and their families, volunteers and the community at-large towards successful outcomes for program participants.

Educational Requirement and Experience:

  • Must have a High School Diploma or GED 
  • Two years’ experience working directly in the human service field. 
  • Proficient in Excel, Word, and Power Point and able to learn Access-based client management systems.

Job Requirements:

  • Must be able to operate a variety of equipment such as computer, printer, fax machine, copier, calculator, telephone, audio-visual equipment, etc.
  • Must be able to sit, stand, walk, reach, bend and lift up to 10 pounds.
  • Must be able to communicate in Standard English both orally and in writing.
  • Must be able to give and receive information electronically.
  • Must be able to communicate successfully and pleasantly with school and central office personnel, students, and families.
  • Must be able to work cooperatively with others.
  • Must be able to respond positively to supervision and accept suggestions for improvement.
  • Must be able to understand and interpret written and verbal instructions.
  • Must keep accurate and up to date records.
  • Must comply with state and district regulations, policies and procedures.
  • Must have reliable transportation.
