Job Description - Admin- Navigator


KCARC is seeking a compassionate, organized, and proactive Navigator to coordinate all aspects of participant admissions, transfers, and discharges for our Residential and Day Programming services. This vital role involves working closely with individuals served, their families or advocates, case managers, and internal departments to support successful transitions and ensure all service plans align with each person’s life vision. 

 

Essential Responsibilities:

  • Serve as the primary point of contact for all matters related to admissions, transfers, and discharges, including individuals transitioning to long-term care facilities.
     
  • Liaise with case managers to oversee and manage budget adjustments and updates for individuals served.
     
  • Distribute referral packets to appropriate internal teams and track the progression of each case.
     
  • Maintain professional relationships with individuals served, families, advocates, and guardians, offering support and guidance throughout each transition.
     
  • Ensure timely and accurate completion of admission, transfer, and discharge documentation in line with organizational policies.
     
  • Update and manage documentation files and communicate changes with the relevant departments.
     
  • Respond to referrals through the BDDS portal and assist families in navigating the waiver application process.
     
  • Collaborate with external agencies such as CPS, APS, and the Health Department when necessary.
     
  • Follow up with individuals and families within 30 days of successful placement or transition into KCARC services.
     
  • Participate in surveys and audits, and promote organizational values through professionalism, empathy, and integrity.
     
  • Provide internal support to KCARC departments as needed and contribute to community support initiatives.

Requirements

Preferred Experience:

  • Background in human services or social work strongly preferred.
     
  • Experience with waiver programs and working within interdisciplinary teams is a plus.

Qualifications: 

  • Must possess a valid driver’s license, reliable transportation, and provide proof of automobile insurance for use during working hours.
     
  • Strong interpersonal and communication skills, both verbal and written, with the ability to engage compassionately and professionally with individuals served, families, and external partners.
     
  • Demonstrated ability to be tactful, empathetic, and culturally sensitive in diverse situations.
     
  • Proven ability to work independently while managing multiple responsibilities, as well as contribute effectively in a team-oriented environment.
     
  • Excellent organizational and time-management skills, with attention to detail and the ability to follow through on tasks.
     
  • Proficiency in Microsoft Office Suite (Word, Excel, Outlook, etc.) and general computer literacy required.
     
  • Ability to analyze and interpret budgets, service plans, and referral documents.
     
  • Strong problem-solving skills and a commitment to maintaining confidentiality and ethical standards.
     
  • Demonstrated initiative and leadership in navigating complex systems or services.
     
  • Flexibility and willingness to adapt to changes in procedures, regulations, and service needs.
