Admissions Screener

Rate: $27.00/hour to $32.00/hour

Full-Time Monday-Friday 8am-4:30pm

The McGuire Group is looking to add an Admissions Screener to its team. In this role, you’ll travel to healthcare facilities and referral sources to assess prospective residents and help ensure smooth, appropriate admissions into the facility. RESPONSIBILITIES:

• Travel to hospitals and healthcare facilities to evaluate prospective residents for admission


• Gather patient information and complete comprehensive admission screens utilizing EMR systems and communication with discharge planners, residents, and families


• Coordinate with hospital discharge planners regarding discharge potential and timelines


• Screen, interview, and assess prospective residents and families to determine admission eligibility


• Provide information and guidance to residents and families throughout the admissions process


• Coordinate admissions with interdisciplinary teams, referral sources, and receiving facilities


• Complete admission and readmission paperwork accurately and efficiently


• Maintain HIPAA, infection control, safety, and corporate compliance standards


• Perform additional duties as assigned by supervisor

REQUIREMENTS:

• Preferred but not necessary Licensed Social Worker, LPN, RN, Therapist, or Bachelor’s Degree in a health-related field  


• Prior healthcare experience required, preferably in long-term care


• Strong understanding of medical terminology required


• Experience with chart review, insurance knowledge, admissions, long-term care, or subacute care preferred


• Valid NYS Driver’s License and reliable daily transportation required


• Strong communication, organizational, time management, and problem-solving skills


• Professional, dependable, and able to maintain confidentiality
