The Otterbein SeniorLife Admissions Central Intake Coordinator will work remotely as part of a self-empowered work team collaborating with patient referral sources and facility leadership. Otterbein is an innovative leader in Long Term care and this position focuses on building census in our industry leading Greenhouse Models. .Responsibilities:

  • Act with a sense of urgency to lead and facilitate the time between referral for admission from various sources to admission decision through collaboration with facility team
  • Enter all referrals into CRM and attach referral packet from hospital
  • Evaluate each referral for Clinical and Financial Viability for admission and take appropriate action based on process parameters
  • Develop and maintain strong relationships with all internal and external customers to yield an increased share of satisfied patient referrals from targeted referral partners
  • Complete or ensure completion of a Patient Assessment and Collaborate with facility to determine eligibility for admission within established time frames
  • Obtain prior authorization for Managed Care or confirm 3 day QHS for MCR.
  • Obtain 7000 or complete PAS (Pre-Admission Screen)
    • Generate Admissions Agreement prior to Admission, ensure completion (remote) within 72 hours / 3 business days of any new admission or re-admission.
  • Ensure discharge orders are uploaded into CRM
  • Ensure communications/notifications are sent to appropriate departments
  • Verify and update current demographic information
  • Complete necessary reports on referrals and admissions and status in the process
Qualifications:
  • Ability to work independently as part of a self-empowered team
  • Communicate effectively in verbal, written and listening modes
  • Act with a sense of urgency to drive results
  • Critical thinker to solve problems across a multitude of platforms and systems
  • Exhibit a high degree of personal accountability
  • Prioritize/multi-task with competing obligations and take appropriate action
  • Identify opportunities for improvement in personal and process enhancements and take action
  • Anticipate customer needs and meet/exceed them in order to deliver exceptional customer service
  • Collaborate and build strong relationship with multiple levels in the organization and with customers
  • Think and act with the end in mind

Education: High School Diploma/GED Required. Strongly prefer Associates or Bachelors Degree

Experience:

  • Prefer 3 or more years of related work experience in Long Term Care or other Healthcare Setting
  • Subject matter expert in long term care payers
  • Referral Management – Electronic Medical Records, Customer Relationship Management Software
  • Customer Service Skills
  • Critical thinking skills and time management
  • Must be able to prioritize/multi task
  • Skilled at relationship building and management remotely
