Admissions Liaison

Position Summary:

The Health Services Admission Liaison manages the admissions process for all (short-term, long-term, and per diem) admissions to the Health Centers at both Crosslands and Kendal at Longwood. This role serves as a key point of contact for prospective residents, families, and referral sources, ensuring a smooth and efficient admission experience.

This is a part-time position, working 20 hours per week.

Responsibilities:

• Oversee the admissions process for Health Centers, including responding to referrals via phone, fax, and email.


• Maintain accurate referral logs and conduct tours for prospective residents and families.


• Coordinate admission requirements, including medical, insurance, and financial qualifications.


• Verify insurance coverage and communicate details to residents or responsible parties.


• Serve as liaison between referral sources, residents, families, and the interdisciplinary team.


• Complete administrative tasks such as preparing admission paperwork, contracts, and distributing documents to appropriate staff.


• Track occupancy, admissions, discharges, and related statistics to support Health Center goals.


• Build relationships with referral sources and promote per diem opportunities.


• Foster teamwork and inclusiveness in the admission decision-making process.

Qualifications:

• Bachelor’s Degree and/or minimum of three years of healthcare experience.


• Strong verbal and written communication skills with excellent customer service abilities.


• Knowledge of admission requirements for skilled nursing facilities or personal care homes.


• Ability to multi-task and work collaboratively with residents, families, referral sources, and internal teams.


• Self-directed and proactive approach to work.
