Admissions Liaison

Essential Functions:

• Responds to requests for onsite assessments and prioritizes internal inquiries to expedite admission to Trio Healthcare facilities.


• Proactively seeks potential referrals from hospital case managers, discharge planners, physicians and other professional referral sources.


• Builds strong professional relationships with key referral partners, including hospital case managers and discharge planners, physicians, and others capable of referring patients to Trio facilities.


• Communicates daily with the Chief Marketing Officer, Central Intake team members and facility admission and marketing directors on referrals, admissions, and discharges.


• Identifies new external referral sources, schedules on-site meetings, and professionally educates on the SNF criteria for admissions and intake.


• Provides ongoing and follow-up communication to referral sources, Central Intake and facility personnel, as well as Trio senior leadership.

• Adheres to Trio Healthcare’s policies and procedures, ethical standards, corporate compliance standards and professional code of conduct. Conducts proper pre-admission patient evaluations in a timely manner.


• Maintains assigned market territory. Develops, implements, evaluates, and refines external efforts based upon data, trends, market needs, and facility needs.


• Coordinates specific marketing and public relations activities such as hospital meetings, educational events, participation in professional networking events, etc. as assigned.


• Effectively tracks admission activity, maintain referral profiles, and ensure compliance with company’s standards.


• Participates in new program development initiatives, as assigned.


• Maintains regular attendance in keeping with assigned work schedule.


• Completes required trainings, as assigned.


• Performs other duties as assigned to support overall effectiveness of the organization.

Minimum Job Requirements:
Minimum Education & Experience:

• 3 years healthcare experience required. Experience in skilled nursing facility admissions and marketing preferred. 

Required Licenses, Certifications, and/or Documentation:

• Clinical licensure preferred but not required (RN, LPN)


• Must maintain valid, acceptable driving record, current driver’s license, and insurability.

Required Knowledge, Skills, and Abilities:

• Knowledge of skilled nursing facility operations and procedures.


• Demonstrates an understanding of patient skilled mix, regulatory requirements, reimbursement, etc. that impact overall facility operations.


• Demonstrates the highest degree of customer service and professionalism.


• Demonstrates general computer skills including data entry, word processing, email, and record management.


• Effective organizational and time management skills.


• Ability to work independently.


• Ability to maintain proper levels of confidentiality.


• Ability to work closely and professionally with others at all levels of the organization.
