BASIC PURPOSE:
This position serves as a mechanism of support in the clinical arena as it relates to new admission processing/auditing and general medical clerking.
MAJOR WORK ACTIVITIES:
- Support unit staff in the admission process to include comprehensive study of incoming admission documentation, capturing orders, and creating charts.
- Notifies pharmacy of medication, and orders and creates EMAR and ensures med procurement.
- Evaluates incoming patients to determine the severity of their illnesses and the intensity of services they will require while in the facility.
- Develops initial rapport with clients and their family members or significant others.
- Develops and initiates initial individualized patient care plan, ensure the team implements care plan. Reassesses at least weekly, adjusts as necessary.
- Develops patient specific education plan. Implements initial education to patient. Communicates plan to team and ensure plan is reinforced.
- Supports nursing in care delivery and documentation. Participates in initial care conference.
- Assists in maintaining resident’s medical records and ensures that information on records is accurate and up to date. Audit, purge and file medical record documentation from each chart per schedule.
- Check and maintain PAR levels on Club and Pavilion (these lists will change based on acuity).
- Audit Lab system for all of Rehab to ensure all labs ordered are drawn, processed, communicated to MD with or without new orders and scanned in.
- Providing oversight and support to communication baskets (MD, Nurse & Scans).
- Audit new admissions to ensure everything is complete and meds are here for each new admit.
- Assist Rehab nurse with passing lunch and dinner medications if no admission is process (club/pav).
- Draws labs if needed (during Admission Nurse hours).
- Perform specific work duties and responsibilities as assigned by the Clinical Coordinator, shift supervisor of DON.
- Complete all clinical chart documentation according to best practice at time service rendered, prior to end of current shift, and/or by “late entry” with date, time, and legible signature or initials where appropriate. Documentation that is not time specific/shift specific, i.e. such as MDS, Care Plans, and certain summaries may be completed with signature and date on the day of documentation, completion and/or at time of review.
- Contributes toward an efficient office environment by performing a variety of administrative functions including but not limited to files/charts/coordinates lab work.
- Demonstrates through knowledge and practice, principles of infection control and universal precautions.
- Safeguards patient privacy rights by judiciously protecting confidential information.
- Operates and maintains a variety of office equipment and orders office supplies.
- Collaborate and assist Central Supply Coordinator with regard to unit needs.
- Reports plan-based resident care to licensed and non-licensed nursing personnel in an effective and equitable manner to ensure ongoing compliance with the state and federal regulations for long term care facilities for all admissions.
- Assist in discharge planning with care plan team members.
- Assess quality of care issues on a continual basis through monitoring documentation.
- Collaborate with other departments with regard to the delivery of services to ensure a safe, clean and pleasant environment on the units.
- Respond to emergency calls as necessary.
- Inform Director of Nursing/Unit Manager of major changes and or concerns.
Requirements
KNOWLEDGE, SKILLS AND WORKING CONDITIONS:
- Good geriatric health assessment skills (physical, mental and developmental staging)
- Knowledge of documentation requirements related to timeliness, accuracy and completeness
- Work is performed indoors with controlled temperatures.
- Exposure to some noise and stress
- Able to multitask in a fast paced environment
- Able to evaluate and respond to verbal and non-verbal communication from residents
- HIPPA compliant; observing strict confidentiality
- Read, write and speak English
- Physical exertion required at least 70% of the time
- Intermitted lifting and carrying of objects up to 20 lbs, bending, and pushing of objects up to 50 lbs.
- Exposure to infectious diseases.
- Sufficiently mobile to push wheelchairs, carts and equipment

MINIMUM QUALIFICATIONS:
Education:
Associates Degree in Nursing from an accredited school of nursing.
Licensure/ Certification:
Current North Carolina licensure as Nurse.
Experience:
Two years previous geriatric nursing experience.
An equivalent combination of education and experience may be considered.
