Responsibilities
● Collaborate with team to provide management and delivery of patient care plans
● Meet critical deadlines for charting
● Support patients in Williamsport, PA and surrounding areas

Schedule and Shift Details
● Monday- Friday 8a-5p, 1 weekend every 4-6 weeks, 2 holidays per year
Travel
● Ability to spend ~20% of your day driving to/from patient locations
