Admissions Registered Nurse (RN) - Home Health, Weekends

About the Role:
At Corus Home Health in Albuquerque, we’re looking for an experienced Admissions RN who thrives in a setting where independence is valued and leadership has your back. In this front-line role, you’ll be the trusted clinical voice welcoming patients to our care — assessing needs, guiding transitions, and making sure they feel seen from day one.

Key Responsibilities:

• Complete home-based admission assessments with empathy and clinical expertise
  
• Educate patients and families about care plans, medications, and expectations
  
• Coordinate with physicians, referral sources, and internal teams to ensure seamless starts of care
  
• Ensure timely and accurate documentation in the EMR
  
• Evaluate clinical eligibility for services and establish visit frequencies
  
• Uphold regulatory standards and agency policies during the admission process
  
• Support patients in navigating resources and setting realistic health goals
  
• Participate in staff meetings and ongoing quality improvement efforts

What We’re Looking For:

• Licensed Registered Nurse (RN) in the state of New Mexico
  
• Minimum of 1 year of recent home health experience preferred
  
• Strong assessment and critical thinking skills
  
• Confidence working independently in the field
  
• Excellent communication with patients, families, and interdisciplinary teams
  
• Familiarity with OASIS and EMR systems (experience with myUnity a plus)
  
• Compassionate, organized, and mission-driven
