Job Summary: The Admissions Team Lead is responsible for oversight of the referral and admissions processes that ensure that care and excellent service are made available to Northern Illinois Hospice healthcare partners, patients, and families. Also responsible for implementing strategic and operational plans to increase referrals and admissions of appropriate patients.
Essential Duties and Responsibilities:
- Supervises all aspects of the Intake Department.
- Is responsible for department’s team coordination, scheduling, and evaluation as well as the supervision of the Admissions RN(s), Intake Coordinators), Hospice Liaison(s), and any other positions added at a later date.
- Ensures competencies of department employees and evaluates at least annually, or as needed including on the Perfect Patient Encounter.
- Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws.
- Reviews each referral for thoroughness and appropriateness, including but not limited to understanding lab values, diagnosis terms and disease trajectories and the application of critical thinking skills. Reach out to providers as needed.
- Participates in training and monitoring all team members on the Perfect Patient Encounter.
- Oversees the Intake team to ensure that the referral/admission process is performed in a smooth and coordinated manner.
- Establishes and maintains a positive environment that encourages patient-centric decision-making, quality service, and teamwork, both departmentally and interdepartmentally.
- Evaluates the effectiveness of the referral process through interviews, periodic meetings and provider satisfaction surveys/discussions of health care and community partners. Tracks and manages ongoing departmental quality through review of key performance indicators.
- Ensures adequate staffing levels; creates and monitors monthly employee schedules; assists in ensuring proper orientation of new employees.
- Develops and monitors the operational workflow of the referral and admission process, including staff scheduling, effective processes, and accurate data entry into the electronic medical record (EMR) and reporting, as well as timely follow-up on pending referrals and effective communication with patients and referral sources.
- Communicates consistently with the Director of Clinical Services to determine RN scheduling priorities to ensure a timely in-take process to ensure timely resolution of areas of concern/difficult issues related to patient, health care and community partners, and employees.
- Assists in the screening and interviewing process and makes recommendations.
- Participates new hire orientation, training, evaluating, coaching, and disciplining department employees.
- Ensures mandatory educational needs of the Intake staff are met.
- Assists in maintaining high quality relationships in hospitals, nursing facilities and other healthcare providers, and managed care organizations, cultivating partnerships and timely referrals for services.
- Trains Northern Illinois Hospice staff and referral sources on the admission and referral process.
- Promotes hospice referrals in the health care community and stimulates timely referrals to Northern Illinois Hospice as the provider of choice, while expanding the referral base.
- Works within departmental budget and achieves assigned financial targets.
- Serves on the Management team and helps create and implement the organization’s strategic plan.
- Assists with educational programs for Clinical staff/volunteers, facilities, healthcare professionals, and the community at-large (e.g., educational presentations, in-services, referral marketing, professional and community collaboration, etc.), utilizing a team of staff, volunteers, and non-staff advocates to help the organization achieve goals for market expansion of Northern Illinois Hospice’s services.
- Attends weekly IDG meetings and all other Clinical Department meetings.
- Attends evening huddle; participates in running evening huddle on rotation basis with other Clinical Team Leads.
- Participates in Administrator on Call (AOC) rotation.

The expected base salary range for this role is $90,000 - $120,000. However, final base pay will be carefully determined based on several factors, including education, experience, knowledge, skills, and abilities, as well as internal equity and alignment with market data. Additionally, this role may be eligible for other forms of compensation and benefits.

Requirements
Education: Bachelor’s degree (BSN) preferred.
Training and Licensure:
- Illinois Professional Registered Nurse (RN) license
- Expected CHPN within 2 years of hire
- Valid Illinois Driver’s license and auto insurance
Experience:
- At least 3 years of hospice or home health admissions and/or case management experience or equivalent required.
- Health care marketing experience preferred.
- Understands referral patterns in the service area.
- Knowledge/Skills/Ability:
- Excellent written and verbal communication skills. Understands and models excellent customer service skills and communication.
- Ability to effectively direct and motivate a professional staff.
- Ability to work closely with a variety of people while effectively using problem-solving skills.
