Job Description - Admissions/Hospital RN Float


Admissions/Hospital RN Float

Unity is seeking a Registered Nurse Admission/Hospital Float to join our mission-driven team. This role provides flexibility and variety, rotating between initiating services (admissions) and/or providing care for patients in the hospital setting. This includes completing skilled nursing assessments with admissions, comprehensive, routine, and acuity visits. Flexibility, teamwork, and exceptional communication are a must for the Float nurse to provide high-quality patient care.

Schedule: This position is 36 hours per week, working four days per week from 9:00 am to 6:00 pm. It includes every fourth weekend in the Green Bay hospitals (Bellin, St. Mary’s, St. Vincent), call backup 10–12 times per week, and 1–2 holidays per year.

What You’ll Do:

  • Review information concerning the patient’s medical background 
  • Identify the appropriate decision maker/agent and follow through with Durable Power of Attorney for Healthcare (DPOAHC), activation, and “In Lieu Of” forms
  • Provide an explanation of hospice and palliative programs and answer questions appropriately
  • Review and complete admission forms with the patient/family 
  • Complete a comprehensive nursing assessment, which includes medication reconciliation, and obtain orders as needed
  • Collaborate with members of the medical team to determine appropriate services as needed
  • Notify referral sources and attending physicians of admission to the hospice or palliative program
  • Assess the need for and order supplies and durable medical equipment
  • Enter appropriate information into the electronic medical record and initiate follow-up
  • Assess and complete supporting documentation for the level of care being provided
  • Develop, implement, and review patient care plans
  • Communicate with the primary team and support services to meet the needs of the patient
  • Collaborate on process improvement efforts
  • Participate in focus groups or other specialized work groups
  • Perform related duties as assigned by the supervisor
  • Maintain compliance with all company policies and procedures

Physical & Environmental Requirements:

  • Ability to sit, walk, bend, and lift up to 35 lbs occasionally.
  • Comfortable using computers, phones, and medical equipment.
  • Regular travel between hospital and patient settings as required.

Requirements

  

1. Associate degree in Nursing required, bachelor’s degree preferred.

2. RN license in the state of practice required (RN must practice under the law and regulations governing nursing in the state of practice).

3. Valid WI Drivers’ License, reliable transportation, and proof of current automobile insurance coverage.
