Admissions/Social Services Director

Vaiden Community Living Center is hiring an Admissions/Social Services Director!   

The Admissions & Social Services Director is responsible for developing and maintaining the facility’s census through referral source relationships, community outreach, marketing initiatives, and effective admissions coordination. This role oversees the inquiry and admissions process, evaluates prospective residents for appropriate placement, and promotes a positive experience for residents and their families.

This position also oversees the facility’s social services program, ensuring residents receive the support they need to maintain their physical, mental, and psychosocial well-being in accordance with company policies and applicable state and federal regulations.

Responsibilities

• Build and maintain strong relationships with referral sources through regular communication and follow-up.
• Meet budgeted census goals and maintain appropriate Medicare and private-pay census.
• Develop and implement promotional campaigns, advertising, and community outreach initiatives.
• Coordinate facility tours, community events, and activities that promote a positive image of the facility.
• Monitor competitor activity, services, and marketing strategies.
• Identify opportunities for new services based on community and referral source needs.
• Track referrals, monitor inquiry activity, and identify opportunities to improve admissions and census growth.
• Train team members on referral source relationships, admissions procedures, and census development.
• Manage the admissions process from initial inquiry through resident placement.
• Evaluate prospective residents to determine appropriate placement and coordinate pre-admission assessments.
• Collaborate with the Administrator, Director of Nursing, and other team members regarding clinical and financial considerations.
• Ensure a welcoming transition for new residents and their families.
• Monitor resident, family, and referral source satisfaction through surveys and follow-up communication.
• Review feedback and recommend improvements to administration.
• Maintain regular communication with families and responsible parties in collaboration with facility leadership.
• Coordinate and conduct required social services programs in accordance with company policies and regulatory standards.
• Maintain accurate and complete social services documentation for each resident.
• Ensure documentation meets applicable state and federal requirements.
• Support residents’ physical, mental, emotional, and psychosocial well-being.
• Collaborate with facility leadership and interdisciplinary team members to address resident and family needs.

Qualifications & Requirements

• Strong communication, interpersonal, and relationship-building skills.
• Experience in admissions, marketing, social services, healthcare, or a related field preferred.
• Ability to develop referral networks and support census growth.
• Strong organizational, problem-solving, and time-management skills.
• Ability to evaluate resident needs and coordinate appropriate placement.
• Knowledge of applicable healthcare regulations and documentation requirements preferred.
• Ability to work collaboratively with residents, families, referral sources, and facility leadership.
• Professional, compassionate, and resident-focused approach.
