Adult Nurse Practitioner- Fort Worth, TX

Segue Health, a faith-based medical practice, is looking to hire a Nurse Practitioner to work in the Burleson, Cleburne, and south Fort Worth (below I 20) areas and see patients for in-home transitional care management and treatment after discharge from an acute care facility. The candidate will provide medical consults, review discharge summaries, review progress notes, collaborate with home health or other members of the healthcare team and prescribe medications when necessary.

Must have the ability to travel up to 1 hour away from home. Candidates needed to cover the following areas: Fort Worth and surrounding communities. 

Must have a current medical license to treat within the state of TX and a current Medicare number.

Job Type: Full time strongly preferred.

Salary: $55.00 - $75.00 per hour
