Adult Nurse Practitioner- Sevierville, TN

Segue Health, a faith-based medical practice, is looking to hire a Nurse Practitioner to work in the Sevierville and surrounding areas to see patients for in-home transitional care management and treatment after discharge from an acute care facility. The candidate will provide medical consults, review discharge summaries, review progress notes, collaborate with home health or other members of the healthcare team and prescribe medications when necessary.

Must have the ability to travel up to 1 hour away from home.  Candidates needed to cover the following areas: Sevierville and surrounding communities. Candidates must be able to provide dedicated day(s) each week for seeing patients.

Must have a current medical license to treat within the state of TN and a current Medicare number.
