Title: Adult Targeted Case Manager

Location: Kentucky River Community Care – Perry, Knott, Leslie, Letcher Counties

Employment Type:  Full-Time

Credentials: Bachelors degree in approved Human Services field with 1 year post degree experience working in relevant field. 

Skills: Experience working with SMI/SUD population.  Must have good communication and computer skills.

Primary Functions:

• Must be able to work independently and have excellent time management skills.
• Provide case management to 25 individuals on the case load at all times.  Clients will have SMI/SUD diagnosis and possible chronic health conditions.
• Link and coordinate services for each client with community partners.
• Complete paperwork in electronic medical record.
