Join a mission-driven, community-based elder care program where you can make a meaningful impact every day. Were seeking a compassionate Nurse Practitioner or Physician Assistant to deliver high-quality primary care to older adults in a collaborative, supportive, team-based setting.

What You'll Do:

• Provide comprehensive routine and episodic assessments
• Manage acute and chronic medical conditions
• Conduct home and facility visits as needed
• Educate participants and ensure timely, accurate documentation
• Participate in a limited, shared on-call rotation

What We are Looking For:

• Licensed NP or PA in Massachusetts
• Experience working with geriatric populations preferred
• Strong clinical judgment, communication, and teamwork skills
