Location: Lexington, KY
Schedule:Monday-Friday 8am-5pm; occasional on-call, weeknight afterhours call, and 1 holiday/year

About the role:
The Advanced Practice Provider- Palliative Care promotes advanced practitioner practice by providing comprehensive, quality care to individuals living with a serious illness. The APP will concentrate on alleviating the symptoms and stresses associated with serious illnesses, aiming to enhance the quality of life for patients, caregivers, and families. An App in this position will be tasked with conducting visits for both new and established patients, which may include home visits, as well as visits to facilities and hospitals. In close collaboration with community physicians, Bluegrass Care Navigators’ physicians, our community partners, and the APP will follow established palliative symptom management protocols. The APP plays a crucial role in developing a care plan for patients, families, and caregivers, and ensures clear communication of this plan to Bluegrass Care Navigators’ physicians, the patient’s primary care provider, or specialist as needed. Additionally, the APP may contribute to the development of practice protocols for palliative and end-of-life care or provide education to other clinicians about the palliative care specialty.
Requirements
Is a graduate of a Master’s or Doctoral Degree in a Nursing Program or a Physician Assistant Program. Must hold either ACNP, AGNP, FNP, or PA certification. In-depth, current knowledge in area of specialty and the ability to translate professional and organizational goals into effective practice.
Is currently licensed as a Nurse Practitioner or Physician Assistant through the appropriate State Boards in Kentucky.
Experience is required with current hospice, palliative care, home health, medical, surgical or critical care experience. Experience in Hospice and Palliative medicine preferred. Has experience with providing good pain management. I.V. skills preferred.
Skilled in the ability to cope with family emotional stress and tolerant of individual lifestyles.
Committed to the research process and application to practice.
Ability to organize resources appropriately.
Strong interpersonal skills, communication, collaboration and conflict resolution.
Knowledge of systems and the change process.
Ability to manage multiple priorities simultaneously and effectively handles the emotional stress of the workload.
Complies with accepted professional standards and principles.
Possesses and maintains current CPR certification.
Has excellent observation, good medical judgment and communication skills. Ability to provide good written documentation in a timely manner.
Is self-directed with the ability to work with little supervision.
Is flexible and cooperative in fulfilling role obligation and has ability to work within an interdisciplinary team.
Must be a licensed driver with an automobile in good working order, that is insured in accordance with state and/or organization requirements, with personal auto liability insurance of $100,000/$300,000 in bodily injury coverage.
Team Members will be expected to comply with Bluegrass Care Navigators (BCN) Employee Health, infectious disease, and vaccination policies.
