Advanced Practice Provider - School Based

Mountain Community Health Partnership (MCHP) is seeking a full time Advanced Practice Provider (Physican Assistant or Nurse Practitioner) to join our two middle school School-Based Health Centers (SBHCs) in Yancey County, NC. The care team at these sites include an experienced RN and a Clinical Social Worker who specialized in children and adolescents and is supported by a Family Medicine Physician with board certification in adolescent health. 

MCHP is the largest provider of pediatric (and adult) care in our community and our SBHCs are an essential part of the care we provide. Patient visits are a combination of acute car, well adolescent care and management of chronic health issues for both students and staff both in person and via telehealth. During the summer and school vacations the Advanced Practice Provider will rotate through our four other practices seeing patients from the SBHCs and across our patient panel. 

The ideal candidate will enjoy the complexity of adolescent health care, works well in a multidisciplinary team and recognizes the clear relationship between health and academic achievement. Experience in integrated behavioral health and managing adolescent depression and anxiety is highly desirable.

The Advanced Practice Provider will adhere to evidence-based guidelines as appropriate and participate in quality improvement activities. The Advanced Practice Provider will have a large scope of practice, including direct provision of care. In addition to providing outpatient medical care, duties include, but are not limited to, complying with all MCHP goals for the practice. They must display knowledge of normal signs of human development and the ability to assess and provide age-appropriated and culturally sensitive care. The Advanced Practice Provider must maintain privacy of all patients, employees, and volunteer information, and access information only on a need-to-know basis for business purposes. Finally, they must provide comprehensive, high quality and culturally sensitive rural primary care through a team-based approach.

The Advanced Practice Provider must have completed a Physician Assistant or Nurse Practitioner program from an accredited institution, maintain an active NC license, and be board certified.
