Job Description - Advanced Practice Registered Nurse (APRN)

Heartland Counseling Services is looking for a Full-Time APRN to join our South Sioux City clinic!


 


MAIN FUNCTION:  The Advanced Practice Registered Nurse (APRN) will provide psychiatric and mental health care services to individuals with serious mental illnesses and/or substance use disorders. As a key member of our interdisciplinary team, the APRN will be responsible for conducting assessments, diagnosing conditions, prescribing medication, and collaborating with other healthcare professionals to ensure comprehensive, client-centered care.


 


DUTIES/RESPONSIBILITIES:


·         Conduct comprehensive psychiatric evaluations and mental health assessments for individuals referred to the CCBHC program.


·         Diagnose and treat a wide range of mental health and behavioral health disorders, including co-occurring substance use disorders.


·         Provide treatment planning, ensuring that care plans are individualized and appropriate for each client’s needs.


·         Collaborate with the treatment team to develop and implement holistic care strategies that integrate psychiatric and behavioral health services.


·         Assess, prescribe, and manage psychotropic medications for clients, ensuring safe and effective pharmacological treatment.


·         Monitor medication effectiveness, manage side effects, and make adjustments to prescriptions, as necessary.


·         Provide education to clients and families regarding medications, treatment options, and symptom management.


·         Collaborate closely with multidisciplinary team members, including psychiatrists, social workers, counselors, and case managers, to coordinate care for clients.


·         Participate in team meetings, case conferences, and treatment planning discussions to ensure continuity and coordination of care.


·         Provide guidance and supervision to other clinical staff as needed, sharing expertise in psychiatric care and mental health management.


·         Provide psychoeducation to clients regarding mental health conditions, coping strategies, and available community resources.


·         Offer support in managing mental health symptoms and assist clients in building skills for long-term recovery and wellness.


·         Offer family support and education to enhance the involvement of family members in the treatment process, as appropriate.


·         Ensure accurate and timely documentation of client assessments, diagnoses, treatment plans, progress notes, and medication management in the agency’s electronic health record (EHR) system.


·         Adhere to all regulatory, compliance, and confidentiality requirements, including HIPAA and agency policies.


·         Participate in audits and reviews to ensure quality care and regulatory compliance.


·         Provide crisis intervention and stabilization for clients experiencing acute psychiatric symptoms or behavioral health crises.


·         Assess and manage risk for self-harm, suicide, or harm to others, and implement appropriate safety plans.


·         Collaborate with other healthcare providers and emergency services to ensure client safety during crises.


·         Stay current with best practices, emerging trends, and research in psychiatric nursing, behavioral health, and CCBHC services.


·         Participate in training, conferences, and continuing education opportunities to maintain licensure and enhance clinical skills.


·         Engage in supervision and peer support to maintain clinical competence and self-care.


·         Assist with program development, quality improvement initiatives, and other agency activities as needed.


·         Participate in community outreach and educational events to promote behavioral health awareness and services.


 


REQUIRED SKILL/ABILITIES:


·         Strong knowledge of psychiatric-mental health diagnoses, treatment modalities, and medication management.


·         Ability to collaborate with interdisciplinary teams and provide leadership within the team.


·         Excellent communication skills, both verbal and written, with the ability to effectively engage clients and families.


·         Ability to maintain a compassionate, non-judgmental, and patient-centered approach to care.


·         Strong organizational skills and attention to detail.


·         Ability to use electronic health records (EHR) systems and other technology platforms effectively.


·         Experience in substance use disorder treatment and dual diagnosis care.


·         Knowledge of CCBHC standards and community-based care models.


·         Bilingual skills (preferred but not required).


·         Possess a valid driver’s license and an insurable driving record.


 


EDUCATION & EXPERIENCE:


·         Master’s degree in nursing (MSN) with a focus on psychiatric-mental health (PMHNP) from an accredited program.


·         Current, valid licensure as an Advanced Practice Registered Nurse (APRN) in Nebraska, with certification as a Psychiatric-Mental Health Nurse Practitioner (PMHNP-BC) preferred.


·         Minimum of 2-3 years of clinical experience in psychiatric or behavioral health settings, providing direct care to individuals with serious mental illnesses or substance use disorders.


·         Experience working in a community-based setting, such as a CCBHC or similar behavioral health organization, is preferred.


·         Experience with both individual and group therapy modalities, including crisis intervention, preferred.
