About the role: 

The Advanced Practice Registered Nurse- Post-Acute & Long-Term Care position exemplifies and promotes advanced nursing practice and excellence in patient care.  Provides direct patient care and evaluates patient outcomes within the skilled nursing facility and the long-term care environment and provides primary care to residents in Assisted Living settings. This position works in collaboration with multidisciplinary health care teams to effectively designate processes for the medical, psychosocial, and spiritual care needs of the facility residents. This position will develop updates and evaluate medical treatment plans, medications, and other necessary care.  The Advanced Practice Registered Nurse- Post-Acute & Long-Term Care position maintains their own professional knowledge and skills as an expert and role model in Bluegrass Care Navigators’ advanced nursing practice.   This position will also assist in providing a plan of care for the patient, family, and/or caregiver as prescribed by the physician and in compliance with regulations as established by the State Board of Registered Nursing, the patient, and the policies of Bluegrass Care Navigators.   

Requirements

  • Is a graduate with a master’s degree in a Nursing Program.  APRN certification as Family or Adult Nurse Practitioner.  In-depth, current knowledge in area of specialty and the ability to translate professional and organizational goals into effective practice.   
  • Is currently licensed as an APRN through the State Board of Nursing.   
  • At least three (3) years general nursing experience Is required. Clinical work experience in nursing facility preferred.  
  • Skilled in nursing practice, able to cope with family emotional stress and tolerant of individual lifestyles.  
  • Committed to the research process and application to practice.   
  • Ability to organize resources appropriately.   
  • Strong interpersonal skills, communication, collaboration, and conflict resolution.  
  • Knowledge of systems and the change process.  
  • Ability to manage multiple priorities simultaneously and effectively handle the emotional stress of the workload.  
  • Complies with accepted professional standards and principles.   
  • Understands Bluegrass Care Navigators’ philosophy and principles of death/dying.   
  • Possesses and maintains current CPR certification.   
  • Have satisfactory references from nursing school, previous (or current) employers, and/or professional peers.   
  • Has excellent observation, good nursing/medical judgment and communication skills.  Ability to provide good written documentation in a timely manner.   
  • Is self-directed with the ability to work with little supervision. Is flexible and cooperative in fulfilling the role's obligation and can work within an interdisciplinary team.  
  • Must be a licensed driver with an automobile in good working order, that is insured in accordance with state and/or organization requirements, with personal auto liability insurance of $100,000/$300,000 in bodily coverage. Team members will be expected to comply with Bluegrass Care Navigators (BCN) Employee Health, infectious disease, and vaccination policies.
