JOB SUMMARY:
Connect neighbors with comprehensive wrap-around services. Research and collaborate with nonprofits, churches, and government agencies to connect neighbors with timely resources and supports. Maintain case management notes for each neighbor as required. Comfortable working face-to-face with neighbors in crisis, hearing difficult stories, and maintain empathy when working with individuals whose needs may exceed available solutions. Flexibility in schedule is essential, including availability for occasional evening hours and occasional travel between local facilities.
ESSENTIAL FUNCTIONS:
Case Management- Connect neighbors with community resources such as financial assistance, affordable housing, transportation options, and subsidized childcare. Meet with those in need and maintain detailed case notes. Will work closely with a team of volunteers/ interns. Maintain strict adherence to all systems, policies, and procedures including case management and resource navigation.
Reporting - Maintain detailed case notes in the UNITY database, TECO Agency Portal, Airtable database, and ASO/Children’s Board, as applicable.
Fund Allocation - Accurately determines appropriate funding sources based on program and funding guidelines.
Intern/Volunteer Supervision - Leads, trains, and supports interns and volunteers through hands-on supervision and coaching.
Agency to Agency Collaboration - Liaison between organization and local social service agencies in order to pioneer, develop, and grow a system for social change in unincorporated Hillsborough County.
