Schedule: Monday-Friday 8am-5pm; occasional on-call rotation and overnight travel may be required

**The responsibilities outlined will require travel to any BCN regional office, which may include overnight accommodations.**  

As a BCN Hospice Team member, you will be a part of a team providing care and support to patients and their families who no longer receive treatments to cure their terminal illness, but instead choose care to improve their comfort and quality of life. While this often is a house or apartment, we also provide care at nursing homes, assisted living facilities, and hospitals. If a patient’s care needs are too complicated for home care or require a higher level of attention from nurses and other experts, we can provide care at one of our Hospice Care Centers.

About the role: 

A team member in the Agency Flex Social Worker position provides direct casework and counseling services to the patients and families within Bluegrass Care Navigators’ Hospice Care Program. The Agency Flex Social Worker will provide an assessment of psychosocial problems define treatment goals, and work to alleviate such problems. The services provided by the Agency Flex Social Worker may require visits in a home or nursing home facility anywhere within in the Bluegrass Care Navigators service regions. Therefore, travel may be required, including overnight accommodations at times. The Agency Flex Social Worker serves as part of the interdisciplinary team and shares responsibility for other aspects of patient care except where limited by licensure. This position also plans and coordinates patient care with the primary nurse under the direction of the Regional Counseling Supervisor. 

• Is a graduate of a Bachelor’s or Master’s program in Social Work accredited by the Council of Social Work Education; and is licensed by the state of Kentucky as a Licensed Social Worker (LSW), a Certified Social Worker (CSW), or a Licensed Clinical Social Worker (LCSW). 
• A minimum of two years social service experience in a health care setting or social service agency.  Experience working on an interdisciplinary team preferred.  
• Knowledge of the process of death, dying, and bereavement.  
• Knowledge and basic understanding of the psychosocial and legal operatives in terminally ill patients and their families.  
• Knowledge and basic understanding of the interpersonal dynamics, operative within the working relationships of an interdisciplinary healthcare team.  
• Ability to exercise judgment and discretion in confidential matters.  
• Must be able to make home visits to patients/families in a variety of settings; mobile homes, apartments, homes, nursing facilities and condominiums that may not have elevators.  
• Must be able to drive for extended periods of time with no ill effects.  
• Must be flexible, good written and oral communication skills. Must have the ability to give and receive constructive feedback.  
• Must be a licensed driver with an automobile in good working order, that is insured in accordance with state and/or organization requirements, with personal auto liability insurance of $100,000/$300,000 in bodily injury coverage. 
• Team members will be expected to comply with Bluegrass Care Navigators (BCN) Employee Health, infectious disease, and vaccination policies.
