Key Responsibilities
Clinical Care & Patient Assessment
- Conduct Intake & Triage: Perform comprehensive patient assessments, obtain accurate medical histories, and record vital signs prior to provider examinations.
- Perform Procedures: Administer medications, injections, and immunizations as prescribed. Perform routine clinical procedures such as wound care, dressing changes, EKG administration, and phlebotomy.
- Assist Providers: Prepare patients and examination rooms and assist physicians or advanced practice providers with minor in-office procedures and exams.
Care Coordination & Triage
- Telephone Triage: Assess patient symptoms over the phone, provide evidence-based medical advice within the RN scope of practice, and determine the urgency of care needed.
- Manage Referrals & Follow-ups: Coordinate prescription refills, lab results communication, specialist referrals, and post-visit follow-up care.
Patient Education & Advocacy
- Empower Patients: Educate patients and their families on disease management, preventive care, medication instructions, and treatment plans.
- Discharge Planning: Review after-visit summaries and ensure patients fully understand their next steps before leaving the clinic.
Documentation & Compliance
- Maintain Records: Accurately and promptly document all patient interactions, assessments, and interventions in the Electronic Health Record (EHR) system RPMS.
- Ensure Safety: Strictly adhere to HIPAA regulations, infection control protocols, and clinic safety standards.
Qualifications & Requirements
Education & Licensure:
- Current, unencumbered Registered Nurse (RN) license in any state, the District of Columbia, the Commonwealth of Puerto Rico, or a Territory of the United States.
- Current Basic Life Support (BLS) certification required.
Experience:
- Minimum of 36 months/3 years of nursing experience. Previous experience in outpatient, ambulatory care, emergency, or med-surg settings is strongly preferred.
