AmeriCorps Member Position Description 

Health Care Partnerships Assistant

General Statement of Duties: In support of the mission of The Ohio Association of Foodbanks, members of the Healthy Harvest Corps will support a coordinated statewide effort to prioritize investments in scaling and sustaining food as medicine initiatives. Members will support new, emerging, and expanding services that address hunger as a social determinant of health.

Service Position Type/ Required Minimum Service Hours -  FT/1700 hours

General Service Schedule: 

Our agency’s standard hours are 7am – 5pm M-F.  The member’s typical work schedule will be: eight hours/day with 30-minute lunch, M-F (with occasional weekend events). General breakdown of your time is below.

• 50% Primary activities: Advance the impact of referral partnerships with healthcare providers and Medicaid managed care organizations activities 

• 40% Secondary activities 

• 5% Training at service site

• 5% Training and events through the Community Health and Research team

Primary activities include (minimum 50% of time):

• Participate in monthly Foodbank Champions Team calls with the network  

• Work to improve the effectiveness of referrals from healthcare providers and Medicaid managed care organizations. 

    - Work with the Director of Community Health and Research, the Director of Help Center and Outreach, and the Root Cause Coordinator for Healthcare Programs to improve outcomes associated with referrals made by healthcare partners through UniteUs. This can include recruiting healthy pantries (i.e. Farmacy participants), providing resources and training to Farmacy pantries, and developing materials for neighbors and partners

    - When needed, receive referrals and make outgoing referrals to food pantries and other emergency food providers based on the needs of the individual referred.

    - Complete screenings for additional health-related social needs and make referrals to other social services providers as needed.

    - Follow-up with individuals referred and provide data back to healthcare providers on needs met and services provided.

• Identify opportunities to engage healthcare partners with the local food pantry network for example, through training on food insecurity screening tools (e.g., Hunger Vital Sign) and procedures for making referrals to the local foodbank and member pantries

• Provide support on Food as Medicine programs that utilize referrals, including medically-supportive groceries and Medically Tailored Meals

Secondary activities (up to 40% of time)  

• Support Community Resource Centers (CRCs) by guiding neighbors through shopping

• Support CRC healthy cooking demonstrations and Farmacy shopping hours for referred patients

• Comply with all AmeriCorps program guidelines by maintaining service hours, training hours, and service projects as outlined in the AmeriCorps Service Agreement.

• Attend all required training courses and staff meetings and participate in AmeriCorps service projects.

• Understand and promote the Mission and Vision of the Greater Cleveland Food Bank that values public service and celebrates the positive aspects of diverse backgrounds.

Requirements

Minimum Qualifications:

• High School Diploma or equivalent

• At least 18 years of age

• Ability to pass criminal background check and drug screen

• Working knowledge of Microsoft office software and data entry programs

• Positive and professional written and oral communication skills

Preferred Qualifications:

• A college degree and demonstrated interest in community and public health

• Highly organized and detail oriented

• Experience and comfort with speaking in front of a group

• Ability to work and communicate with people of a variety of backgrounds and skillsets
