General Statement of Job:
This is a dual role position as a wound care nurse and relief advance practitioner registered nurse. This position coordinates treatment to provide comprehensive pressure injury (wound) assessment and plan of care including goals, implementation of treatment, evaluation and outcomes for patients throughout the organization. Provides patient, family and staff education regarding wound management, prevention of pressure ulcers, support surfaces, dressing selections and other therapies. Participates in performance improvement plan of the organization.
Essential Functions:
Manages the facility pressure Injury (wound care) program, completes documentation related to wound management and prepares reports as outlined and as requested.
Ability to perform pressure injury (wound) assessment, reassessment and management for all patients and per policy.
Demonstrates thorough knowledge of staging system of pressure injuries (wounds).
Develops skin care programs to prevent skin breakdown of patients.
Serves as a resource to physicians, nursing and other facility staff. Provides in-services on pressure injury (wound) prevention and pressure injury (wound) care management.
Knowledgeable of medications and their correct administration based on the patient?s clinical condition.
Consult and collaborate with wound care physician and other healthcare professionals/departments as appropriate to provide interdisciplinary approach to the patient?s needs.
Prepares written and oral reports related to the pressure injury (wound care) program.
Participates in the development, implementation, communication, and evaluation of care plans related to pressure injury (wound care) management.
Ability to revise plan of care as indicated by the patient's response to treatment and evaluate overall plan for effectiveness.
Lead the interdisciplinary team in the weekly focus meeting.
Ability to adequately assess and reassess pain. Utilizes appropriate pain management techniques. Educates the patient and family regarding pain management.
Assesses and records residents? medical and surgical history, physical and psychosocial needs.
Compiles and evaluates medical and psychological histories of EJHC residents.
Performs complete physical assessments of residents.
Identifies and evaluates problems, complaints, and symptoms as reported and presented by residents, by resident families, or by staff about the residents.
Initiates treatment and therapies according to the protocols established mutually between the APRN and supervising physician.
Prescribes medications, monitors medications, and alters medication according to the protocols established mutually between the APRN and the supervising physician.
Orders and performs tests and procedures according to the protocols established mutually between the APRN and the supervising physician.
Plans and conducts health education for residents, families and facility staff related to the resident's medical condition, treatment, or prognosis.
Participates in the facility's quality assurance/quality monitoring program.
Participates in the residents plan of care, interdisciplinary team meetings, committee meetings and rehabilitation meetings when necessary.
Utilize EHR to record findings, results, recommendations and orders, timely, according to facility policies and procedure.
Discusses findings as needed with attending physician, consulting staff, and medical service providers.
Reviews the results of tests and procedures, making notations and changes in residents plans or orders according to the protocols established mutually between the APRN and the supervising physician.
Performs mental status exams.
Continually monitors the health care services provided and changes orders as necessary to meet the individual resident/patient needs according to the protocols established mutually between the APRN and the supervising physician.
Contacts the physician for any condition requiring direct evaluation or specific consultation by the physician according to the protocols established mutually between the APRN and the supervising physician.
Interacts with patients family members.
Conducts pre-admission consulting.
Emergency duty may be required of the incumbent that includes working in Red Cross shelters or to perform other emergency duties including, but not limited to, responses to threats or disasters, man-made or natural.

Education:
Degree requirements per the Florida State Board of Nursing for APRNs; Master's Degree preferred.
Experience:
Three (3) to five (5) years previous experience in caring for critically/terminally ill or aging persons required. Three (2) to five (5) years of wound care experience and wound care certification
Certification:
Basic Life Support (BLS) certification required.
Licensure:
Adult-Gerontology NP Certification or Family NP Certification Required. State of Florida, Department of Health Advanced Registered Nurse Practitioner required; must have signed agreement with licensed physician in accordance with Florida law.
Registrations:
APRN Registration required.
