Position Overview:

The Home Health Care Registered Nurse is responsible for providing and documenting skilled nursing care in accordance with the developed care plan and physicians’ orders for each individual client while adhering to confidentiality standards and professional boundaries at all times.

Responsibilities of a Registered Nurse RN

• Conduct nursing initial assessments and reassessments, developing individualized care plans for clients while notifying personnel of any changes


• Collaborate with physicians, therapists, and other care team members to coordinate and optimize client care plans.


• Medication management, administration


• Case management


• Monitor client’s condition and notify appropriate personnel of any changes


• Perform Wound care / simple and complex (wound vacs).


• Perform IV home infusion (peripheral IVs, Ports, PICC/central line), vitals, medication administration (Abx, TPN, IVIG, and other medications), blood draws, insertions, therapy management, and PORT access/maintenance following sterile procedures.


• Educate clients and families on disease management, IV care, Port handling and wound care and VAC device maintenance, in a home setting.


• Document client care accurately and timely in electronic system.


• Respond effectively to medical emergencies and escalate concerns as needed.


• Supervises, Educates and coaches other nursing personnel/care-giving staff.


• In Home Health Care RN will see an average of 4-7 clients per day in the clients’ homes. The daily or weekly coverage will be based on type of client care.

Requirements of a Registered Nurse RN

• Valid In-State RN License in South Carolina or Multi-State License


• 1-2 yrs of In-Home nursing experience required


• IV home nursing experience


• Current CPR / BLS card
