Job Description - ASC Medical Record Clerk

PURPOSE 


Under the direction of the Business Office Manager, the Medical Records Clerk is responsible for organizing and maintaining the medical record system. Monitors patients’ medical records status and location throughout the surgery center.  Carries out all duties while maintaining compliance and confidentiality and promoting the mission and philosophy of the organization.


ESSENTIAL FUNCTIONS 



  • Responsible for organizing and maintaining the medical record system.

  • Monitors patients’ medical records status and location throughout the surgery center.

  • Ensures all paper and electronic records are complete, properly identified and signed.

  • Scans/files laboratory, radiology and other reports in appropriate sections of the electronic medical record within prescribed time period.

  • Copies, retrieves and tracks medical records requests.

  • Assembles charts for peer reviews.

  • Conducts chart audits on a daily basis.

  • Responsible for tracking dictations to assure all cases are dictated. Follows up with physicians regarding any missing dictation of cases.

  • Maintains compliance with all relevant laws and regulations regarding the handling of medical records.

  • Provides regular communication and feedback to supervisor.

  • Responsible for achieving performance excellence through skills of organization; creates successful workflow processes; reliable in all areas of work.

  • Demonstrates respect and regard for the dignity of all patients, families, visitors, and fellow employees to ensure a professional, responsible, and courteous environment.

  • Communicates with others, including physician offices, insurance companies, patients, co-workers, and others about insurance benefits and costs associated to procedures.

  • Adheres to all company policies, including but not limited to OSHA, HIPAA, compliance, and Code of Conduct.

  • Regular and dependable attendance.

  • Follows the core competencies set forth by the Company, which are available for review on ARC SharePoint.

  • Performs other duties as assigned.


QUALIFICATIONS


Education and Experience 


Required: 



  • High school diploma or GED.

  • Two (2) or more years’ experience with medical records.


Preferred: 



  • ASC experience


Certificate/License 



  • Microsoft Office Specialist certifications preferred.


 


Knowledge, Skills and Abilities 



  • Excellent verbal and written documentation and communication skills.

  • Ability to work variable hours and be flexible to meet the needs of the center’s daily surgery schedule.

  • Knowledge of medical terminology.

  • Excellent customer service skills.

  • Excellent computer and keyboarding skills. Must be adept at using EMR, Microsoft Word and Windows, communicate by e-mail and use scheduling software.

  • Excellent interpersonal and problem solve skills.

  • Ability to work in a team environment.

  • Ability to engage others, listen and adapt response to meet others’ needs.

  • Ability to perform job duties in a professional manner at all times.

  • Ability to align own actions with those of other team members committed to common goals.

  • Ability to understand, recall, and apply oral and/or written instructions or other information.

  • Ability to organize thoughts and ideas into understandable terminology.

  • Ability to apply common sense in performing job.

  • Ability to demonstrate a proactive approach to problem identification and solutions.

  • Ability to demonstrate appropriate time management skills, prioritization and task completion.

  • Ability to handle multiple assignments or pieces of work at one time. The capability of working within a stressful environment under consistent deadlines, pressures and interruptions.

  • Ability to cope with and manage change.

  • Ability to exhibit an exceptional grasp of required skills including: detail-oriented, commitment to accuracy, able to anticipate needs before they become critical.


Work Schedule: Monday- Friday
