Position Summary
The Assistant Administrator serves as a central administrative and care coordination resource supporting resident services across multiple assisted living homes. This role combines case management principles, family and resident relations, admissions coordination, administrative support, and care operations to ensure residents receive seamless, high-quality care throughout their stay.
Working closely with residents, families, hospitals, home health agencies, hospice providers, physicians, and internal care teams, the Assistant Administrator supports and helps to facilitate admissions, transitions of care, documentation, regulatory compliance, and ongoing communication while supporting day-to-day operational needs.
This position is ideal for someone who enjoys balancing administrative responsibilities with resident advocacy, problem-solving, and healthcare coordination in a fast-paced environment.
Essential Responsibilities
Resident Case Management & Care Coordination
- Assist with transition planning between hospitals, home health, hospice, and Fresno Guest Homes.
- Assist with admissions, physician checklists, resident re-appraisals and assessments in the Electronic Healthcare Record (EHR).
- Communicate with hospitals, physicians, home health agencies, hospice providers, pharmacies, and other healthcare partners.
- Prepare hospice licensing letter and facility hospice consents.
- Obtain monthly Hospice Plans of Care and training from agencies residents are enrolled with.
- Assist Executive Administrator (EA) during licensing visits, annual visits, and complaints.
- Support Alternative Living Waiver (ALW) assessments and required documentation.
- Prepare and organize admission packets and resident documentation.
- Maintain accurate resident records and ensure timely completion of required documentation.
Admissions & Administrative Operations
- Support and prepare with admissions by completing necessary paperwork, admissions signings, new home binders, resident signage and other operational materials.
- Upload and organize electronic resident documentation.
- Assist with licensing preparation and regulatory readiness.
- Maintain organized electronic and paper filing systems.
- Support billing processes and manage monthly resident spend on personal items.
- Coordinate annual operational projects and recurring administrative initiatives.
Care Operations Support
- Maintain daily operational tracking via Slack including resident documentation, medication-related logs, and clinical communication tools.
- Assist with documentation supporting insulin tracking, bowel movement logs, PRN documentation, refusals, and other operational records.
- Coordinate medication refill requests with appropriate clinical personnel.
- Coordinate and manage annual pharmacy vaccine clinics.
- Support inventory management of medical supplies, personal care items, and operational supplies.
Family & Community Communication
- Build positive relationships with residents and families through timely, professional communication.
- Provide updates during hospitalizations or significant care transitions.
- Coordinate communication between families and interdisciplinary care teams.
- Support excellent customer service throughout the resident experience.
- Work with and support one on one with residents, specific resident needs, behaviors, and support to residents and families.
Regulatory & Operational Support
- Assist with licensing inspections and regulatory compliance preparation.
- Help maintain employee and operational records required for licensing.
- Ensure documentation is accurate, organized, and readily available for audits or inspections.
- Support continuous process improvement initiatives within Care Operations.
- Manage and coordinate End of the Year tasks: annual physician reports, annual resident re-appraisals, facility CSM binders, and others as needed.
- Investigate resident incidents and complaints, complete all necessary Incident Reports and preparations of CA state required forms and documents.
Qualifications
- High School Diploma or GED required.
- Bachelor’s degree in Social Services or Social Work preferred.
- Minimum 2 years of experience in one or more of the following:
- Skilled Nursing Facility (SNF)
- Assisted Living
- Home Health
- Hospice
- Hospital Case Management
- Healthcare Administration
- Experience coordinating care across multiple providers.
- Excellent organizational and multitasking skills.
- Strong interpersonal and communication skills.
- High attention to detail.
- Ability to prioritize multiple competing tasks.
- Proficient in Microsoft Office, Slack, Google Workspace, Adobe Acrobat, and electronic health record systems.
- Ability to maintain confidentiality and exercise sound judgment.
- Valid California Driver's License and reliable transportation.
- Criminal background clearance required.
- Pre-employment physical and TB clearance required.
Preferred Qualifications
- Case Management experience in a Skilled Nursing Facility, Assisted Living, Hospital, or Home Health setting.
- Experience coordinating admissions or discharge planning.
- Knowledge of California RCFE regulations.
- Familiarity with hospice and home health workflows.
- Experience working with electronic health records and healthcare documentation.
