The Assistant Patient Care Advocate supports patients, families, and care teams by facilitating communication, addressing concerns, and ensuring a positive patient experience. This role assists with patient coordination, documentation, and follow-up while advocating for patient needs and compliance with organizational policies and healthcare regulations.

FUNCTIONS: 

  1. Identifies key referral sources, including appropriate physician offices in order to help educate the referral source to the services and benefits of the Hospice Program for their terminally ill patients.
  2. Presents the advantage of the Medicare Hospice Benefit to referral sources as well as patients and families. This requires a thorough understanding of the Medicare Hospice Benefit, the Medicare Long Term Benefit and the Medicare HMO Benefit.
  3. Describes and explains hospice services to potential patient/family and elicits responses to questions regarding patient fears, physical limitations and attitude toward hospice
  4. Speak with patient/family regarding financial obligations and insurance coverage, using knowledge of hospice fees as well as potential insurance reimbursement.
  5. Confer with the Intake Department Manager/team in circumstances of potential admission where reimbursement sources appear not to cover cost of care to ascertain whether the patient may be admitted on a charitable basis.
  6. Help to develop opportunities for and assist with in-service presentations to hospitals and other appropriate referral sources regarding hospice care and the indications for a hospice referral.
  7. Participates in community health fairs as assigned by Director of Business Development. 
  8. Can navigate through EMR, email, referral database and other electronic communication. 
  9. Attends daily stand up and weekly marketing meetings along with monthly all staffs. 
  10. Maintains a master copy of the Admission Consent Forms and prepares packets as needed.
  11. Comprehends the hospice local coverage determination criteria set by Medicare and can help identify medically eligible patients for hospice. 
  12. Prepares weekly marketing supplies and/or bereavement baskets as needed.  
  13. Assists Director of Business Development and Intake Department Manager on daily tasks that need to be accomplished. 
  14. Supports the Field Coordinators/AC’s/Senior AC’s in the field with presentations, co-marketing opportunities, and patient/family meetings to review/sign hospice consents. 
  15. Successfully develops relationships in the field with key referral sources.
  16. Understands the importance of follow-up skills to further enhance relationship development and displays these skills as well as creativity.
  17. Assists Senior AC’s with identifying possible contract relationships for the hospice program of care.
  18. On call back up rotation at least 1 weekend per month. 
  19. Other duties as assigned. 

Requirements

QUALIFICATIONS: 

  1. Excellent communication skills, both verbal and written. 
  2. Office experience in health care setting preferred. 
  3. Computer knowledge and good organization skills. 
  4. 2 years of sales and marketing experience in healthcare preferred. 
  5. A strong desire to work as an integral member of the admissions/marketing team.
  6. This position reports directly to the Director of Business Development
