Job Description - Assistant Patient Care Scheduler





Description

Assistant Patient Care Scheduler will play a crucial role in coordinating and scheduling Hospice Aides and LPNs to ensure smooth patient care operations. The scheduler’s responsibilities include responding to scheduling requests, coordinating with clinical managers, and maintaining accurate patient care scheduling records.

What you will do:

  • Responds promptly to all scheduling requests for Hospice Aides and LPN’s
  • Assign and coordinate Hospice aide/LPN schedules based on established policies and procedures. 
  • Communicate regularly with the DPS/Clinical Support Services Director regarding scheduling priorities, resource sharing, and unfilled requests
  • Proactively contact newly admitted patients to establish HHA/LPN schedules.
  • Adhere to all state and federal regulations governing LPN/HHA Scope of Practice.
  • Facilitate all communication between Hospice Aides, LPNs, Clinical Support Services Director, and DPS.
  • Promotes and enhances communication between Hospice Aides, Case Managers, and DPS/ Clinical Support Services Director
  • Reviews schedules for Hospice Aides/LPNs ensuring they meet documentation requirements daily according to agency policy.
  • Receives approval from DPS/ Clinical Support Services Director before scheduling Hospice Aide schedule change or overtime.
  • Maintains electronic schedule on a daily basis and communicate changes as they occur daily via e-mail, and tiger connect.
  • Completes Hospice Aide Documentation Audit monthly as assigned by VP of Mission Quality
  • Enters all Hospice Aide projected visits in electronic schedule according to Hudson Valley Hospice guidelines. 
  • Schedules Continuous Care/Crisis Care after request received and approved by DPS.
  • Participates in Hospice Aide in-services and education.
  • Participates in orientation program for Hospice New hire aides as needed.

  • Ability to effectively communicate verbally (in person and by phone) and in writing.
  • Ability to work effectively and collaboratively as a team member. 
  • Ability to prioritize and handle multiple tasks assignments efficiently in a fast-paced environment.
  • Highly developed organizational skills.

Work Environment:

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.  

  • Primarily work in an office environment and conditions, as needed.
  • Travel by car in the field, as needed.
  • Routine duties involve potential for direct exposure to blood or body fluids.
  • Ability to carry up to twenty-five pounds of equipment and/or supplies.
  • Ability to work on call evenings, weekends, and holidays

Education & Experience we are looking for:

  • Minimum of one year scheduling/clerical experience in healthcare required
  • Minimum High School Diploma 
  • Hospice experience preferred.
  • Excellent computer skills and must be proficient in Microsoft office suite, spreadsheets & data entry.
  • Valid NYS Driver’s License. 

Hours: Monday-Friday 9:00am-5:00pm (Weekend and Holiday coverage rotation)

Pay:

FLSA: Non-Exempt
