Job Description - Assistant Team Leader


JOB DEFINITION:


Work with the CN Guidance FACT Team to deliver services to those individuals who are diagnosed with severe mental illness and whose needs have not been met by more traditional service delivery approaches.



EDUCATIONAL REQUIREMENTS:


A master's degree or higher in social work, psychology, rehabilitation counseling, or related field. Licensure with the NYS Education Department in a specific field is preferred. Limited permits are required if not licensed.



ADDITIONAL REQUIREMENTS:



  • Current New York State Driver's License

  • Clean DMV Record

  • Proof of Motor Vehicle Insurance



EXPERIENCE REQUIRED:


Two years' experience working with the chronically mentally ill (Masters level). Competency in substance abuse, family support, or vocational rehabilitation must be demonstrated by at least one year of training and/or experience in integrated mental health and substance abuse assessment and treatment.



DUTIES AND RESPONSIBILITIES:



  1. In absence of Team Leader, direct and supervise staff activities, lead team organizational and service planning meetings and provide direction to staff regarding individual cases.

  2. Perform routine team duties, and have lead responsibility for integrating dual-recovery treatment, family support or vocational/employment training with the tasks of other team members.

  3. Responsible for providing Assessment and Service Planning, Quality Assurance and Improvement Processes, Utilization Review, incident reporting and coordination between ACT and other systems.

  4. Engage in performing assessments, case management, handling crisis intervention, and discharge planning.

  5. Maintains case records and completing documentation on time and in a matter in compliance with federal, state and agency regulations.

  6. Provides direct, Telehealth, face-to-face and/or collateral services to recipients either in the FACT Office or in the community in places and situations where problems arise.

  7. Must be available (along with other members of the FACT Team) on an as needed basis 24 hours a day, 7 days a week, including responsibility for periodic pager coverage.

  8. Assists recipients in developing appropriate expectations from treatment.

  9. Strengthens the ability of the recipient to understand and utilize therapy and find resources after treatment has ended through proper discharge planning and termination process.

  10. Exchanges information with other agencies that benefits the recipient, or ensures compliance with regulations and coordination of services.

  11. Utilizes and understand the value of supervision, which increases practice skills, promotes better services for the recipient and preserves accountability to the Agency.

  12. Support the use of data in clinical supervision to inform decision-making

  13. Identify and implement quality improvement trainings and activities as part of professional development goals

  14. Ability to cultivate a culture of inclusion for all employees that respects their individual strengths, views and experiences. A Culture that makes better decisions, drives innovation, and delivers better business results.

  15. Adherence to all safety protocols and procedures to ensure a safe working environment for all employees. Demonstrate a commitment to maintaining a safe work environment by following established safety guidelines.

  16. Health and Safety Awareness: Include any relevant knowledge or experience regarding health and safety protocols to ensure a safe work environment for all employees.

  17. Support the use of data in clinical supervision to inform decision-making

  18. Identify and implement quality improvement trainings and activities as part of professional development goals

  19. Ability to cultivate a culture of inclusion for all employees that respects their individual strengths, views and experiences. A Culture that makes better decisions, drives innovation, and delivers better business results.

  20. Adherence to all safety protocols and procedures to ensure a safe working environment for all employees. Demonstrate a commitment to maintaining a safe work environment by following established safety guidelines.

  21. Proficiency with virtual collaboration tools or managing remote teams if applicable.

  22. Other duties as assigned.
