The goal of and Assisted Living nurse is to provide direct patient care, administer medications, and treatments according to physician orders. Perform assessments and appropriate documentation of such in resident's individual medical record.

This opportunity is a full-time, 2nd shift (3pm-11:30pm) position including every other weekend and some holidays. If hired on as an LPN, this would be a union eligible position. Westminster Village offers a competitive wage with an annual increase. The wage scale for the positions are as follows:

  • RN: $38 to $40 per hour
  • LPN: $30.45 to $32.45 per hour

Job Duties:

  • Assists the Director of Assisted Living with the 24-hour accountability for resident care.
  • Performs direct patient care to residents according to physician’s orders and in conformance with the recognized nursing procedures, established standards, and administrative policy.
  • Administer medications and treatments as prescribed.
  • Assists with the administration of specialized therapy including infusion therapy, PICC lines, respiratory, tracheostomy care, wound care, and treatments as prescribed.
  • Performs admissions including assessments and documentation on all residents.
  • Participate in the care planning process and assist in maintaining the accuracy of care plans between scheduled reviews.
  • Works in collaboration with Physicians, Pharmacists, Dietitian, Therapists, Activities and Social Services as well as families and other authorized persons and disciplines involved in the care of the resident to facilitate communication and execution of the total plan of care.
  • Observes and encourages infections control measures in accordance with facility policy as well as the rules and regulations governing infection control practices.
  • Observe and promote the facility goal of “Restraint Free Environment” and assists others in finding alternative measures.
  • Observes and promotes the facility goal of “Limited Lift Environment”.
  • Protects patient confidentiality in all aspects of care and all communication of information, computer communication, telephone communication, and direct communication MUST be handled in strict confidence according to the laws which govern Patient Confidentiality.
  • Performs and documents assessments of residents, including behavior monitoring, physical assessment, and response to specific medications, etc.
  • Assists physician in examination of resident or with treatments being performed by the physician.
  • Responds to emergency calls to the Village or Willows if needed.
  • Assists with principles and goals of Restorative Nursing by accountability in restorative procedures as indicated by the plan of care and performed by CNA’s, PCA’s, and Memory Care Assistants.
  • Performs other duties as assigned by the Director of Assisted Living or Memory Care Coordinator.
  • Familiarize oneself with location and use of firefighting equipment, alarms, and disaster procedures.
  • Attend educational meetings planned for this position.
  • Abides by all facility policies including but not limited to the Abuse Policy.
  • Assists with daily staffing in the absence of scheduler and/or Director of Assisted Living.

Requirements

High School Diploma, GED or equivalent. Must be a graduate in good standing of an accredited school of nursing. Current and unencumbered license to practice in the State of Illinois as a Registered Professional Nurse. Work experience in Gerontological Nursing is strongly desired. Medical/Surgical experience is acceptable. Otherwise, recommended by Director of Nursing Services. Working knowledge of healthcare delivery in the Geriatric, Rehabilitation, or Skilled Nursing setting. Knowledge of the aging process, pharmacology, assessment of the elderly, and practical application of clinical interventions. Desire to work in a skilled/rehabilitation setting is required. Current CPR certification is required.?

RN: $38 - $40 / hour LPN: $30.45 - $32.45 / hour
