Essential Functions
Customer Service:
· Build and maintain strong, trust-based relationships with key referral partners through proactive communication and consistent, high-touch support.
· Serve as the primary liaison for high-volume and high-priority referral sources, ensuring exceptional customer service and responsiveness.
· Collaborate with internal teams to ensure seamless onboarding, service coordination, and ongoing support for referral partners.
· Identify opportunities to enhance partner engagement and satisfaction, addressing concerns promptly and effectively.
· Track partner activity and feedback to inform service improvements and strengthen long-term referral relationships.
· Represent the organization in a professional and approachable manner, fostering positive partner perceptions and increasing referral volume.
· Serve as a strategic point of contact for high-profile or sensitive client situations requiring leadership-level consultation and support.
· Assists, as needed, in complex case discussions or escalated meetings at the request of staff or clients, providing guidance, resolution planning, and executive oversight.
· Collaborate with internal teams and external partners to ensure aligned communication and decision-making in high-stakes scenarios.
· Represent organizational leadership in key meetings to reinforce commitment to service quality, accountability, and client satisfaction.
· Provide follow-up communication and direction after high-level meetings to ensure execution of agreed-upon action plans.
Management:
· Lead contract management and negotiation processes with external partners, including Armsmear and CND, ensuring favorable terms and compliance with organizational standards.
· Conduct rate negotiations with providers to align service costs with budgetary and strategic objectives.
· Support the hiring process by coordinating onboarding logistics for new staff, facilitating a smooth transition into the organization.
· Champion staff development through ongoing training, supervision, and culture-building initiatives that promote engagement, morale, and retention
· Curate internal newsletters and organize thoughtful gestures (e.g., notes, gifts, acknowledgments) to enhance team connection and recognize contributions
Business Operations:
· Participate in development and implementation of new systems, offering insight to ensure operational alignment and usability.
· Collaborate with cross-functional teams to test, refine, and roll out new technology solutions, ensuring provider needs and compliance requirements are fully addressed.
· Provide strategic oversight of the program’s financial operations, including reconciliation processes and invoice review to ensure accuracy and timeliness.
· Participate in monthly finance meetings to report on program spending, address discrepancies, and support budgetary planning and forecasting.
Business Growth and Development:
· Proactively seek out and secure new group contracts to expand Sage’s care management services into untapped markets.
· Develop and execute strategic outreach plans to establish Sage’s presence in new territories.
· Build relationships with potential partners, referral sources, and organizations to introduce Sage’s offerings and identify collaboration opportunities.
