Associate Discharge Planner at Kadlec Regional Medical Center

Under the general supervision of the Manager of Clinical Resource Management, the Associate Discharge Planner supports the department in discharge planning implementation and administrative tasks.  The Associate Discharge Planner implements the delegated discharge plan under the supervision of the care manager, registered nurse, or provider.

• Full time, Day Shift, 40 working hours weekly

Required Qualifications:

• Three (2) years of clinical or administrative experience in healthcare industry

Preferred Qualifications:

• Bachelor's Degree in Social Work
