FTE: 1.0, Shift: Day, Schedule: Salaried, Monday - Friday, 0800-1630

Hybrid role

This role supports MultiCare’s Bed Readiness Program through system‑wide referral coordination, placement, and active case management of patients in the INW and Yakima markets. The position requires an in‑person Case Manager serving the Yakima or Inland Northwest geographic areas.

Position Summary

To provide a single point of focus for rehabilitation patients as they move throughout the continuum of care to assure smooth, appropriate, coordinated, timely and resource sensitive transitions and assuring those services provided are in the best interest of the patient and family. This position operates independently, working cooperatively and collaboratively with patients, families’ health care providers, payors, and management. Work situations require substantial clinical competency and specialized knowledge, judgment, and skill. This position requires self-direction, motivation, organization, and interpretation of information relating to a wide variety of policies, procedures, and programs.

Work situations are frequently non-routine, requiring analysis and exhibiting independence and the use of discretion and good judgment in decision-making. Internal contacts are at all levels and require establishing rapport and developing strong working relationships. External contacts include outside organizations, physicians.

Requirements

  • Graduate from an accredited school of the specific discipline
  • Currently licensed as a Physical Therapist, Occupational Therapist, Speech Pathologist or Social Worker in the State of Washington or Registered Nurse license (RN) in Washington State or multi-state licensure endorsement (MSL)
  • Minimum three (3) years of clinical experience as a member of an interdisciplinary rehabilitation team is required
  • Consideration may be given to internal candidates not meeting minimum requirements

Pay and Benefit Expectations

We provide a comprehensive benefits package, including competitive salary, medical, dental and retirement benefits and paid time off. As required by various pay transparency laws, we share a competitive range of compensation for candidates hired into each position. The pay scale is $79,830.40 - $114,878.40 USD. However, pay is influenced by factors specific to applicants, including but not limited to: skill set, level of experience, and certification(s) and/or education. If this position is associated with a union contract, pay will be reflective of the appropriate step on the pay scale to which the applicant’s years of experience align.
