Job Description - Behavioral Health Care Coordinator


BEHAVIORAL HEALTH CARE COORDINATOR: We are seeking a compassionate and organized Behavioral Health Care Coordinator to join our team and support clients through seamless coordination of care. This position plays a critical role in ensuring that all internal and external services are effectively aligned to meet the individual needs of each client. The Care Coordinator will manage referrals, facilitate communication across service providers, and ensure a cohesive and clearly documented plan of care for each client on the assigned interdisciplinary team.  The ideal candidate will work collaboratively with behavioral health professionals, families, and community partners to support transitions between levels of care and ensure follow-through on team recommendations, contacts, and required documentation. By maintaining up-to-date care plans and supporting service coordination, the Care Coordinator helps deliver exceptional, client-centered care across all phases of treatment and support.

PAY RANGE/BENEFIT PACKAGE: 
Salary Range: starting at $39,222.86 up to $54,127.54 annually - placement above minimum salary is based on experience.

REQUIRED QUALIFICATIONS:
  • High school diploma/GED with at least two years’ experience in behavioral health service delivery with the target population, preferably in a publicly funded setting 
  • Customer service experience 
  • Community Health Worker (CHW) certification through MDHHS or ability to complete training within the first six months of hire 
PREFERRED QUALIFICATIONS:
  • Associate’s degree in human services  
  • CCBHC experience 
  • Client-level systems coordination experience 
  • Working knowledge of mental health and/or substance use disorders and/or intellectual/ developmental disabilities. 
  • Lived experiences with mental illness/developmental disabilities/co-occurring disorders/substance use disorders.  
GENERAL RESPONSIBILITIES:
  • Initiate, follow-up and document internal and external care coordination contacts and activities.
  • Communicate effectively with individuals and entities involved in a client’s med clinic services, adapting communication, as necessary.
  • Establish working relationships with internal and external individuals and entities frequently involved in client services and care, particularly primary care offices.
  • Facilitate referrals and warm handoffs in partnership with clients, particularly to primary care offices for ongoing behavioral health medication management.
  • Assure interdisciplinary team (IDT) members are updated on care coordination activities verbally and through EMR documentation.
  • Request and follow-up on medical records from primary care and specialty offices.
  • Assure required release of information documents are completed and filed in the EMR according to established regulatory, funding, and agency best practice standards prior to any care coordination activities.
  • Maintain updated contact information for all relevant individuals and entities in each consumer’s EMR.
  • Actively take part in scheduled and impromptu IDT meetings and discussions to assure exceptional, coordinated, and effective care and service delivery.
  • Coordinate contacts to ensure consistent messaging, non-duplication, and focus on overall treatment goals and objectives.
  • Assure clients are aware of care coordination activities and the intent of such activities, directly or via team members.
  • Monitor responsiveness to internal and external care coordination activities and notify supervisor of trends.
  • Coordinate with internal medical clinic, particularly med clinic care coordinator to ensure proper PCP and other medical professional coordination.  
  • As part of a care coordination team, executes the following processes/tasks as assigned by supervisor:
    • BTC process/paperwork 
    • NABD process (in connection with customer services) and submission or NABD’s across assigned teams 
    • Follow up with clients/coordinate with the clerical team on missed appointments and ensure rescheduling 
    • Support Children’s Outpatient, Home based or Wrap-Around Clients 
    • Support Adult Outpatient Clients 
    • Support Recovery Management Clients 
    • Support Adult Case Management Clients 
  • Perform other duties as assigned.

POSITIONS TO BE FILLED: 
  • Two full-time positions to fill. (40 hours/week)
