Behavioral Health Case Manager/Care Specialist - (Onsite)

Zarephath is a behavioral health agency that works with children and families experiencing behavioral health challenges. Care Specialist help clients birth to 17 with a behavior health diagnosis to gain access to community services. Care Specialist work with the guardians/caregivers and other agency staff to provide well rounded care plan for each client.

Responsibilities and Duties

• Shifts: Monday – Friday –- 8:30am-5pm (onsite) 
• Assessing clients’ physical and mental wellness, needs, preferences, and abilities
• Develop tailored care plans.
• Working with clients, family and friend support networks and health care professionals to put care plans in place.
• Recording clients’ progress including charting referrals, home visits and other notable interactions.
• Evaluating clients’ progress periodically and adjusting as needed to improve outcomes.
• Following up with discharged clients to ensure they were satisfied with services, and they are still in good physical and mental health.
• Attend and conduct CFT meeting for assigned caseload.

  Qualifications and Skills
  - Must be 21 years of age
  - Minimum 2-year experience working in behavioral health field working with youth and families.
  - Possess a valid Arizona driver's license and be insurable under the agency's automobile policy.
  - Possess a valid Arizona Fingerprint Clearance Card or be able to obtain one.
  - Must be computer literate with knowledge of Outlook and Microsoft Office programs (i.e. Word, Excel)
  
  Pay Rate: $36,000-$40,000 per year.
