Job Description - Behavioral Health Community Health Worker (MAT/SUD Dept.)

Description

Join MCHC's Dora Street Health Centers as a Behavioral Health Community Health Worker (MAT/SUD Dept.) and make a meaningful impact every day. In this role, you’ll provide compassionate support and care coordination for patients with mental health and substance use diagnoses. You’ll connect patients with essential resources, facilitate behavioral health referrals, and work closely with teams like ECM, BH, and MAT/SUD to improve health outcomes.

Your responsibilities will include patient outreach, health education, advocacy, and maintaining accurate EMR documentation. If you’re passionate about helping others and creating lasting change, MCHC is the place for you!

Requirements

Required Qualifications:

  • High school diploma or equivalent.
  • Strong interpersonal and motivational interviewing skills.
  • Have and maintain a valid California Driver’s License. Must carry liability/bodily injury on his/her automobile(s) used for work.

Preferred Qualifications:

  • Associate’s degree in a behavioral health, health, or social services-related field.
  • Completion of a formal Community Health Worker training program.
  • Bilingual (English /Spanish)
  • Experience in behavioral health, case management, community outreach, and working with underserved populations.
  • Familiarity with social determinants of health and community resources.
  • Experience with electronic medical record (EMR) systems, such as Epic.
